摘要
[目的]探讨一期骨囊肿病灶刮除灭活、同种异体骨与甲泼尼龙琥珀酸钠混合植骨、病灶引流减压的三联综合疗法治疗儿童单纯性骨囊肿的临床疗效。[方法]对接受三联综合疗法治疗的27例儿童单纯性骨囊肿进行回顾性分析。评价临床及影像结果。[结果]所有患者顺利完成手术,随访时间平均(35.2±4.5)个月。随术前、术后3个月及末次随访的时间推移,VAS评分[分,(6.8±1.3),(1.2±0.4),(0.9±0.2),P<0.001]、局部压痛[例,无/轻/中/重,(3/5/8/11),(21/5/1/0),(25/2/0/0),P<0.001]、患肢活动度[例,正常/轻度受限/显著受限,(4/16/7),(21/4/2),(26/1/0),P<0.001]均显著改善。影像方面,随时间推移,Neer评级无显著变化(P>0.05)。术后2个月X线片见皮质开始增厚,髓腔透光性降低;术后9个月见骨皮质基本正常,髓腔无透光区;术后1年骨皮质厚度正常,无膨胀,髓腔无透光区。[结论]三联综合疗法治疗儿童单纯性骨囊肿临床疗效满意。
[Objective]To explore the clinical efficacy of a triad combined therapy,including bone cyst lesion curettage and inactiva⁃tion,bone allografting mixed with methylprednisolone succinate,and lesion drainage and decompression for simple bone cyst in children.[Methods]A retrospective study was conducted on 27 children who had the bone cysts treated by the abovesaid triad combined therapy.The clinical and imaging data were evaluated.[Results]All patients had operations performed successfully,and followed up for a mean of(35.2±4.5)months.As time progressed before surgery,3 months after surgery,and the last follow-up,the VAS[score,(6.8±1.3),(1.2±0.4),(0.9±0.2),P<0.001],local tenderness[n,none/mild/moderate/severe,(3/5/8/11),(21/5/1/0),(25/2/0/0),P<0.001],and limb movement[n,normal/mild restriction/severe restriction,(4/16/7),(21/4/2),(26/1/0),P<0.001]all significantly improved.In term of imaging,there was no significant change in Neer grade over time(P>0.05).The X-rays showed that the cortical bone began to thicken and the transparency of the medullary cavity decreased 2 months postoperatively,the cortical bone was basically normal without transparent area in the medullary cavi⁃ty 9 months after surgery,the thickness of the cortical bone was normal,with no expansion,and no transparent area in the medullary cavity 1 year after surgery.[Conclusion]The triad combined therapy is clinically effective for treating simple bone cysts in children.
