摘要
先天性马蹄内翻足(congenital talipes equinovarus,CTEV)是一种常见的儿童足部畸形,目前其病因仍不明确,治疗后复发率高,仍是临床的挑战。本文全面综述了其评估及治疗方法。评估方法涉及Pirani、Dimeglio等主观评估法,国际马蹄足畸形研究学组(International Clubfoot Study Group,ICFSG)评分、美国足踝关节矫形协会(American Orthopaedic Foot and An⁃kle Society,AOFAS)量表等综合评估法,以及步态分析、电生理检测等新兴方法。非手术治疗涵盖Ponseti疗法、French功能疗法、电生理疗法等,其中Ponseti疗法是标准方法。手术治疗适用于重度、复发及保守治疗效果不佳患者,包括软组织松解术、肌腱转移术、骨性手术和Ilizarov技术等。未来基因技术、多领域融合,如身心健康与社会关注整合,有望推动治疗变革。
Congenital talipes equinovarus(CTEV)is a common foot deformity in children,with unclear etiology and high recreant rate after treatment in current,which remain challenges for clinicians.This paper provides a comprehensive review of its assessment and treat⁃ment methods.The assessment encompasses subjective assessment methods,such as Pirani and Dimeglio methods,and comprehensive eval⁃uations,such as International Clubfoot Study Group Evaluation System(ICFSG)and American Orthopaedic Foot and Ankle Society(AO⁃FAS),as well as the emerging approaches like gait analysis and electrophysiological detection.Non-surgical treatments include Ponseti ther⁃apy,French functional therapy,electrophysiological therapy,etc,among which Ponseti therapy is the standard one.Surgical treatment is suit⁃able for patients with severe,recurrent,or poor responses to conservative treatment,including soft tissue release,tendon transfer,bony sur⁃gery,and Ilizarov technique.In the future,genetic techniques and multi-domain integration,such as physical-mental health and social inte⁃gration are expected to drive therapeutic changes.
