胸部X线摄影中辐射剂量估算与风险分析

摘要 目的:应用两种放射剂量估算系统评估胸部X线摄影检查中辐射剂量及其风险的变化趋势。方法:收集本院近一年的胸部X线摄影检查辐射剂量数据与受检者信息,按年龄分为0~<1岁、1~<5岁、5~<10岁、10~<15岁、15~<18岁和成人共六组,统计曝光参数,使用Radtech放射剂量分析系统和PCX-MC软件估算有效剂量、器官剂量和辐射风险。采用Wilcoxon检验比较两种方法所获取的有效剂量,采用Kolmogorov-Smirnov检验比较两者变化趋势。结果:共收集有效数据胸部正位片63303例、侧位片9332例。Radtech统计的有效剂量正位片为0.0161(0.00955,0.02109)mSv,侧位片为0.04264(0.01666,0.07315)mSv。按年龄和摄影条件分组后,两种方法估算的各年龄段有效剂量数据Wilcoxon检验差异无统计学意义,且变化趋势一致。Radtech系统和PCX-MC软件估算成年女性胸部正位摄影乳腺器官剂量分别为0.010721 mGy、0.00893 mGy,子宫剂量分别为0.000123 mGy、0.00007 mGy。Radtech系统胸部正位X线摄影检查辐射致癌几率为2.6×10^(-7);PCX-MC软件计算胸部正位X线摄影检查成年男女受检者辐射致癌风险分别为6.88×10^(-7)、1.07×10^(-6)。结论:胸部X线摄影检查是一种相对安全的放射检查方法,辐射风险较低。通过有效放射剂量管理和优化摄影参数,有潜力进一步降低受检者的辐射剂量,提高放射防护水平。 Objective:The purpose of the study was to evaluate the variation trends of radiation dose and its associated risks in the X-ray chest radiography using two radiation dose estimation systems.Methods:The radiation dose data from the X-ray chest examinations and patient information were collected from our hospital in the past year.All patients were divided into six groups based on age:0~1 year,1~5 years,5~10 years,10~15 years,15~18 years and adults.Statistical analysis of exposure parameters of each group was conducted.Effective dose,organ dose and radiation risk were estimated using Radtech radiation dose analysis management system and PCX-MC software.The Wilcoxon test was employed to ascertain whether there were any significant differences between the effective doses obtained by the two methods.Additionally,the Kolmogorov-Smirnov test was used to evaluate the similarities and discrepancies between the trends observed in the two datasets.Results:A total of 63,303 chest frontal and 9,332 lateral radiographs were collected as effective data.The effective dose measured by Radtech was 0.0161(0.00955,0.02109)mSv in the frontal and 0.04264(0.01666,0.07315)mSv in the lateral.After grouping according to age and photographic parameters,there was no significant statistical difference in the effective dose data of each age group estimated by the two methods,and the variation trends were consistent.The breast organ dose of adult female chest radiography estimated by Radtech system and PCX-MC software was 0.010721mGy and 0.00893mGy,and the uterine dose was 0.000123mGy and 0.00007mGy,respectively.The radiation carcinogenesis probability of X-ray frontal chest radiography estimated using the Radtech system was 2.6×10^(-7).The radiation carcinogenic risk of adult male and female subjects calculated using PCX-MC software in X-ray frontal chest radiograph was 6.88×10^(-7)and 1.07×10^(-6),respectively.Conclusion:Chest radiography is a relatively safe radiological examination method with low radiation risk.It has the potential to further reduce the radiation dose to the examinee and improve the level of radiological protection through effective radiation dose management and optimisation of the radiographic parameters.
