摘要
目的探讨低频经皮神经电刺激与平衡针灸腰痛穴对基层患者腰部训练疼痛的疼痛缓解与腰椎功能改善的效果。方法前瞻性选取2024年10月至2025年4月解放军总医院第一医学中心收治的腰部训练疼痛患者72例,根据随机数表法将患者分为低频经皮神经电刺激组和平衡针灸腰痛穴组,每组36例。两组患者分别接受相应治疗,疗程为2周。比较两组一般资料;比较治疗前后视觉模拟评分法(VAS)评分及Oswestry功能障碍指数(ODI)评分。结果重复测量方差分析显示,VAS评分时间×组别交互效应显著(F=4.231,P=0.021)。治疗前、随访7 d,两组VAS评分比较,差异无统计学意义(P>0.05)。治疗后、随访3 d、随访7 d,两组VAS评分均低于治疗前,差异有统计学意义(P<0.05);治疗后、随访3 d,平衡针灸腰痛穴组VAS评分[(1.19±1.69)分、(1.14±1.29)分]低于低频经皮神经电刺激组[(2.06±1.58)分、(1.81±1.41)分],差异有统计学意义(P<0.05)。重复测量方差分析显示,ODI评分时间×组别交互效应显著(F=3.894,P=0.035)。治疗前,两组ODI评分比较,差异无统计学意义(P>0.05)。治疗后、随访3 d、随访7 d,两组ODI评分均低于治疗前,且平衡针灸腰痛穴组低于低频经皮神经电刺激组,差异有统计学意义(P<0.05)。结论低频经皮神经电刺激与平衡针灸腰痛穴均能有效缓解腰部训练疼痛、改善腰椎功能,且平衡针灸腰痛穴在起效速度与疗效强度方面优于低频经皮神经电刺激。
Objective To compare the effects of low-frequency transcutaneous electrical nerve stimulation and balance acupuncture at the lumbago acupoint on pain relief and lumbar function improvement in patients with training-induced low back pain at the primary care level.Methods A total of 72 patients with training-induced low back pain admitted to First Medical Center of PLA General Hospital from October 2024 to April 2025 were prospectively selected and divided into a low-frequency transcutaneous electrical nerve stimulation(TENS)group(36 cases)and a balance acupuncture at the lumbago acupoint group(36 cases)using a random number table.Both groups received their respective treatments for a course of 2 weeks.General data were compared between the two groups.Visual Analogue Scale(VAS)and Oswestry Disability Index(ODI)scores were compared before and after treatment.Results Repeated-measures analysis of variance showed a significant time×group interaction effect on VAS scores(F=4.231,P=0.021).There was no statistically significant difference in VAS scores between the two groups before treatment and at the 7-day follow-up(P>0.05).After treatment and at the 3-day and 7-day follow-ups,VAS scores in both groups were significantly lower than before treatment(P<0.05).After treatment and at the 3-day follow-up,the VAS scores of the balance acupuncture at the lumbago acupoint group[(1.19±1.69)points,(1.14±1.29)points]were significantly lower than those of the low-frequency TENS group[(2.06±1.58)points,(1.81±1.41)points](P<0.05).Repeated-measures analysis of variance showed a significant time×group interaction effect on ODI scores(F=3.894,P=0.035).There was no statistically significant difference in ODI scores between the two groups before treatment(P>0.05).After treatment and at the 3-day and 7-day follow-ups,ODI scores in both groups were significantly lower than before treatment,and the scores of the balance acupuncture at the lumbago acupoint group were significantly lower than those of the low-frequency TENS group(P<0.05).Conclusion Both low-frequency transcutaneous electrical nerve stimulation and balance acupuncture at the lumbago acupoint can effectively alleviate training-induced low back pain and improve lumbar function.Balance acupuncture at the lumbago acupoint is superior to low-frequency transcutaneous electrical nerve stimulation in terms of onset speed and therapeutic intensity.
