摘要
目的探讨国产NATON机器人辅助下膝关节内侧单髁置换手术的精准性、安全性和早期疗效。方法回顾性选取2022年7月至10月在解放军总医院第四医学中心行国产NATON机器人辅助膝关节内侧单髁置换的膝关节前内侧骨关节炎患者27例,其中男6例,女21例;年龄50.0~77.0(62.2±4.5)岁;身体质量指数18.2~27.5(23.2±2.5)kg/m^(2)。手术均使用国产NATON机器人辅助操作,采用LINK SLED膝关节单髁假体(德国LINK公司)。所有患者均进行单侧手术,比较术前、术后及末次随访时膝关节内翻角度、胫骨及股骨假体置入角度、牛津膝关节评分(OKS)和美国膝关节协会评分(KSS)。结果患者随访18.0~22.0(20.2±1.0)个月。末次随访时,胫骨假体冠状位内翻角为-1.2°~4.4°(1.1°±0.7°);股骨假体冠状位内翻角为-4.7°~9.4°(3.5°±2.1°)。末次随访时,患侧膝关节OKS评分由术前(33.4±2.0)分下降至(12.2±4.2)分,KSS临床评分由术前的(45.0±11.0)分提高至(88.0±3.0)分,KSS功能评分由术前的(44.0±12.0)分提高至(77.0±10.0)分,差异均有统计学意义(P<0.01)。随访期间所有患者均未发生骨折、感染、下肢静脉血栓形成等严重并发症。结论国产NATON机器人能够辅助膝关节单髁置换手术安全操作并实现假体的精准安装,无严重并发症,短期疗效满意。
Objective To investigate the accuracy,safety and early efficacy of the domestic NATON robotic-assisted medial unicompartmental knee arthroplasty.Methods A retrospective analysis was performed on 27 patients with anteromedial osteoarthritis of the knee who underwent domestic NATON robotic-assisted medial unicompartmental knee arthroplasty at the Fourth Medical Center of PLA General Hospital from July to October 2022.There were 6 males and 21 females,aged 50.0-77.0(62.2±4.5)years;body mass index ranged from 18.2 to 27.5(23.2±2.5)kg/m^(2).All surgeries were performed with the assistance of the domestic NATON robot,and the LINK SLED unicompartmental knee prosthesis(LINK GmbH,Germany)was used.Unilateral surgery was performed in all patients.The knee varus angle,tibial and femoral component placement angles,Oxford Knee Score(OKS),and Knee Society Score(KSS)were compared before surgery,after surgery,and at the last follow-up.Results All patients were followed up for 18.0–22.0(20.2±1.0)months.At the last follow-up,the coronal tibial component varus angle ranged from-1.2°–4.4°(1.1°±0.7°);the coronal femoral component varus angle ranged from-4.7°–9.4°(3.5°±2.1°).The OKS of the affected knee decreased from(33.4±2.0)preoperatively to(12.2±4.2)at the last follow-up;the KSS clinical score increased from(45.0±11.0)preoperatively to(88.0±3.0);and the KSS functional score increased from(44.0±12.0)preoperatively to(77.0±10.0).All differences were statistically significant(P<0.01).No severe complications such as fracture,infection,or lower extremity venous thrombosis occurred during the follow-up period.Conclusion The domestic NATON robot can assist in the safe performance of medial unicompartmental knee arthroplasty and achieve accurate prosthesis placement,with no severe complications and satisfactory short-term efficacy.
