多模态超声评估肌痛型颞下颌关节功能紊乱患者二腹肌生物学特征

Multimodal Ultrasound Evaluation of Biological Characteristics of Digastric Muscle in Patients with Myogenous Temporomandibular Disorders

摘要 目的通过多模态超声获取颞下颌关节功能紊乱综合征(TMD)患者及正常人二腹肌的生物学特征,为治疗肌痛型颞下颌关节紊乱提供参考。方法选取30名正常人(男15名,女15名)为对照组,48例单侧肌痛型TMD患者(男11例,女37例)为观察组。常规超声观察两组受试者二腹肌前腹形态及内部回声差异,测量二腹肌前腹厚度,应用剪切波弹性成像技术获取两组受试者双侧二腹肌前腹剪切波速度。结果对照组双侧二腹肌前腹厚度和剪切波弹性速度比较差异无统计学意义(P>0.05)。观察组患者患侧二腹肌前腹内部结构紊乱,肌束膜和肌外膜增厚;患侧二腹肌前腹厚度和剪切波速度大于正常侧(P<0.05)。结论肌痛型TMD患者患侧二腹肌前腹厚度增加,剪切波速度增大,肌肉硬度增加。利用多模态超声测量二腹肌前腹厚度、剪切波速度可能成为临床诊断肌痛型TMD的新方法。 Objective To acquire the biological characteristics of the digastric muscle in patients with myogenous temporomandibular disorders(TMD)and normal individuals in line with multimodal ultrasound,offering new reference for the treatment of myogenous temporomandibular disorders.Methods The selection of 30 healthy individuals(15 males and 15 females)as control group was conducted,and the choice of 48 patients with unilateral myogenous TMD(11 males and 37 females)as observation group was also carried out.Conventional ultrasound was employed to make the observation of anterior belly of digastric muscle morphology and internal echogenicity of participants in both groups,and the thickness of anterior belly of digastric muscle was measured.Shear wave elastography was applied to acquire the shear wave velocity values of the bilateral anterior belly of digastric muscle of participants in both groups.Results No statistically significant differences were found in the thickness and shear wave elastic velocity of the bilateral anterior belly of digastric muscle in control group(P>0.05).The internal structure of the affected side’s anterior belly of digastric muscle was disordered in observation group,with thickening of the muscle fascia and epimysium.The thickness and shear wave velocity values of the affected side’s anterior belly of digastric muscle were greater than those of the normal side(P<0.05).Conclusion Patients with myogenous TMD demonstrated increased thickness,elevated shear wave velocity values,and greater muscle stiffness in the affected side’s anterior belly of digastric muscle.Multimodal ultrasound measurement of anterior belly of digastric muscle thickness and shear wave velocity may serve as a novel clinical diagnostic approach for myogenous TMD.
