摘要
糖尿病防治是我国当前公共卫生面临的重大难题,提升疾病知晓率、治疗率与控制率迫在眉睫。石家庄市第二医院通过国家糖尿病标准化防控中心(DPCC)建设项目,整合专科资源、构建医联体网络、强化基层能力、推行全病程管理,形成"筛、防、管、治、教"闭环防控模式,探索慢性病防治中心视角下糖尿病标准化防控的区域化实施路径。有效推动了糖尿病防控重心下沉与关口前移,提升了管理效率及服务同质化水平,初步实现患者住院率、医疗费用双下降及基层诊疗能力的提升,为区域糖尿病防控及"健康中国"行动落地提供实践参考。
Diabetes prevention and treatment remain a major public health challenge in China,underscoring an urgent need to improve disease awareness,treatment rates,and control rates.Through the implementation of the National Diabetes Prevention and Control Center(DPCC)program,Shijiazhuang Second Hospital has integrated specialized resources,established a medical consortium network,strengthened primary care capacity,and promoted whole-course disease management.These efforts have resulted in the development of a closed-loop prevention and control model encompassing"screening,prevention,management,treatment,and education",and have enabled the exploration of a regional pathway for standardized diabetes prevention and control from the perspective of a chronic disease center.This initiative has effectively shifted the focus of diabetes care downward to the community level and forward to earlier stages,improved management efficiency and service homogenization,and achieved preliminary reductions in both hospitalization rates and medical costs,while enhancing primary care diagnostic and therapeutic capabilities.The experience provides a practical reference for regional diabetes prevention and control and supports the implementation of the"Healthy China"initiative.
