摘要
目的:探讨腹腔镜下全子宫切除术治疗高级别宫颈上皮内瘤变的临床效果。方法:选取南京鼓楼医院集团仪征医院2022年1月—2025年1月收治的高度恶性子宫颈上皮瘤样病变患者60例,根据手术入路方式分为腹腔镜组(30例)和传统开腹组(30例)。传统开腹组行经腹全子宫切除术,腹腔镜组行腹腔镜下全子宫切除手术。比较两组治疗效果。结果:腹腔镜组手术时间长于传统开腹组(P<0.001),术中出血量少于传统开腹组(P<0.001),肛门排气时间、下床活动时间及住院时间短于传统开腹组(P<0.001);术后1、3 d,腹腔镜组CD4+/CD8+高于传统开腹组(P<0.05);术后3 d,腹腔镜组C反应蛋白、白细胞介素-6、肿瘤坏死因子-α、白细胞计数及中性粒细胞比例低于传统开腹组(P<0.001);两组并发症总发生率比较,差异无统计学意义(P>0.05)。结论:腹腔镜下全子宫切除术治疗高级别宫颈上皮内瘤变的效果较好,能够在一定程度上改善围术期指标,保护免疫功能,减轻炎性反应,且安全性良好。
Objective:To explore the clinical effect of laparoscopic total hysterectomy in treatment of high-grade cervical intraepithelial neoplasia.Methods:A total of 60 patients with high-grade malignant cervical intraepithelial neoplasia admitted to Yizheng Hospital of Nanjing Drum Tower Hospital Group from January 2022 to January 2025 were selected.According to the surgical approach,they were divided into the laparoscopic group(30 cases)and the traditional laparotomy group(30 cases).The traditional laparotomy group underwent total abdominal hysterectomy,and the laparoscopic group underwent laparoscopic total hysterectomy.The therapeutic effects were compared between the two groups.Results:Compared with the traditional laparotomy group,the laparoscopic group had a longer operation time(P<0.001),less intraoperative blood loss(P<0.001),shorter time to anal flatus passage,shorter time to ambulation,and shorter length of hospital stay(P<0.001).On 1 d and 3 d after surgery,CD4+/CD8+in the laparoscopic group was higher than that in the traditional laparotomy group(P<0.05).On 3 d after surgery,C-reactive protein,interleukin-6,tumor necrosis factor-α,white blood cell count and neutrophil ratio in the laparoscopic group were lower than those in the traditional laparotomy group(P<0.001).There was no statistically significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion:Laparoscopic total hysterectomy achieves better effects in the treatment of high-grade cervical intraepithelial neoplasia,and can improve perioperative indicators to a certain extent,protect immune function,relieve inflammatory reaction,with good safety.
