血清IL-8、sB7-H3、CXCL16水平在大叶性肺炎患儿疗程中的变化

摘要 目的:通过分析血清IL-8、sB7-H3、CXCL16水平在大叶性肺炎患儿疗程中的动态变化及患儿发生治疗无效的影响因素,探讨血清IL-8、sB7-H3、CXCL16对大叶性肺炎患儿治疗效果的预测价值。方法:回顾性选取我院2023年1月-2025年1月106例大叶性肺炎患儿为观察组,依照1:1选例原则,选取同期健康体检儿童106例为对照组,比较2组血清IL-8、sB7-H3、CXCL16水平,比较观察组不同治疗时间下血清IL-8、sB7-H3、CXCL16水平,单因素分析治疗2个疗程后治疗无效的影响因素,logistic回归分析各因素是否为大叶性肺炎患儿治疗无效的独立危险因素。结果:观察组血清IL-8、sB7-H3、CXCL16水平高于对照组,差异有统计学意义(P<0.05);治疗前血清IL-8、sB7-H3、CXCL16水平>治疗1个疗程>治疗2个疗程>治疗3个疗程,差异有统计学意义(P<0.05);治疗无效患者血清IL-8、sB7-H3、CXCL16水平高于治疗有效患者,差异有统计学意义(P0.05);治疗无效患儿胸腔积液、肺外并发症、心动过速、IL-8≥39.80pg/mL、sB7-H3≥117.16pg/mL、CXCL16≥1.32ng/mL发生率均高于治疗有效患儿,且各指标均为患儿发生治疗无效的危险因素,差异有统计学意义(P0.05)。结论:血清IL-8、sB7-H3、CXCL16水平在大叶性肺炎患儿疗程中变化显著,可为临床预测患儿治疗效果提供参考。 Objective:To analyze the dynamic changes of serum levels of IL−8,sB7 H3 and CXCL16 in children with lobar pneumonia during the course of treatment and the influencing factors of treatment failure,and to explore the predictive value of serum IL—8,sB7-H3 and CXCL16 for the treatment effect of children with lobar pneumonia.Methods:A total of 106 cases of children with lobar pneumonia in our hospital from January 2023-January 2025 were selected as an observation group,and 106 cases of healthy children in the same period were selected as a control group according to the principle of 1:1.The levels of serum IL−8,sB7H3 and CXCL16 in the two groups were compared,and the levels of serum IL-8,sB7H3 and CXCL16 in the observation group at different treatment times were compared.The influencing factors of treatment failure after two courses of treatment were analyzed by univariate analysis,and the independent risk factors of treatment failure in children with lobar pneumonia were analyzed by logistic regression.Results:The serum levels of IL−8,sB7−H3 and CXCL16 in the observation group were significantly higher than those in the control group(P<0.05);The levels of serum IL—8,sB7-H3 and CXCL16 before treatment were>1 course of treatment>2 courses of treatment>3 courses of treatment,and the differences were statistically significant(P<0.05);The levels of serum IL—8,sB7-H3 and CXCL16 in patients with ineffective treatment were higher than those in patients with effective treatment,and the differences were statistically significant(P<0.05);The incidence of pleural effusion,extrapulmonary complications,tachycardia,IL−8≥39.80 pg/mlL,sB7−H3≥117.16 pg/mL,CXCL16≥1.32 ng/mL in children with ineffective treatment were higher than those in children with effective treatment,and all indicators were risk factors for ineffective treatment in children,the differences were statistically significant(P<0.05).Conclusion:The serum levels of IL—8,sB7-H3 and CXCL16 in children with lobar pneumonia change significantly during the course of treatment,which can provide reference for clinical prediction of the treatment effect of children.
