摘要
目的:分析眼底动脉超声血流参数水平与糖尿病视网膜病变(DR)的相关性。方法:选取2022年7月至2024年8月该院收治的138例2型糖尿病患者进行横断面研究,根据有无发生DR将其分为DR组78例、单纯糖尿病(DM)组60例,另选取同期在该院行眼部检查无视网膜病变的60名健康体检者作为对照组。比较三组眼底动脉超声血流参数[眼动脉、视网膜中央动脉、睫状后短动脉的舒张末期流速(EDV)、收缩峰值流速(PSV)、阻力指数(RI),视网膜中央静脉的最大流速(V_(max))、最小流速(V_(min))]水平,分析眼底动脉超声血流参数水平与DR的相关性。结果:DR组、DM组眼动脉、视网膜中央动脉、睫状后短动脉的PSV、EDV水平均低于对照组,且DR组低于DM组,DR组、DM组眼动脉、视网膜中央动脉、睫状后短动脉的RI及视网膜中央静脉的V_(max)、V_(min)水平均高于对照组,且DR组高于DM组,差异有统计学意义(P<0.05);Spearman相关性分析结果显示,眼动脉、视网膜中央动脉、睫状后短动脉的PSV、EDV水平与DR均呈负相关(r<0,P<0.05);眼动脉、视网膜中央动脉、睫状后短动脉的RI及视网膜中央静脉的V_(max)、V_(min)水平与DR均呈正相关(r>0,P<0.05)。结论:眼底动脉超声血流参数水平与DR存在相关性。
Objective:To analyze correlation between fundus artery ultrasound blood flow parameter levels and diabetic retinopathy(DR).Methods:A cross-sectional study was conducted on 138 patients with type 2 diabetes mellitus admitted to the hospital from July 2022 to August 2024.According to the presence or absence of DR,they were divided into DR group(78 cases)and simple diabetes mellitus(DM)group(60 cases).In addition,60 healthy subjects without retinopathy who underwent eye examination in this hospital during the same period were selected as the control group.The ultrasonic blood flow parameters of fundus artery[end-diastolic velocity(EDV),peak systolic velocity(PSV)and resistance index(RI)of ophthalmic artery,central retinal artery and short posterior ciliary artery;the maximum blood flow velocity(V_(max))and minimum blood flow velocity(V_(min))of the central retinal vein]were compared among the three groups.Spearman correlation was used to analyze the correlation between the levels of blood flow parameters of fundus artery ultrasound and DR.Results:The PSV and EDV levels of ophthalmic artery,central retinal artery and short posterior ciliary artery in DR group and DM group were lower than those in control group,and those in DR group were lower than those in DM group;the RI levels of ophthalmic artery,central retinal artery,and short posterior ciliary artery,and the V_(max)and V_(min)levels of central retinal vein in DR group and DM group were higher than those in control group,and those in DR group were higher than those in DM group;and the differences were statistically significant(P<0.05).The Spearman correlation analysis showed that the PSV and EDV levels of ophthalmic artery,central retinal artery and short posterior ciliary artery were negatively correlated with DR(r<0,P<0.05);while the RI levels of ophthalmic artery,central retinal artery and short posterior ciliary artery as well as the V_(max)and V_(min)levels of central retinal vein were positively correlated with DR(r>0,P<0.05).Conclusions:There is a certain correlation between the fundus artery ultrasound blood flow parameter levels and DR.
