呼吸振荡排痰系统对脑梗死合并坠积性肺炎患者血气指标和神经功能缺损恢复情况的影响

The Impact of Respiratory Oscillatory Expectoration System on Blood Gas Index and Neurological Function Recovery in Patients with Cerebral Infarction Combined with Hypostatic Pneumonia

摘要 目的:探讨呼吸振荡排痰系统在脑梗死合并坠积性肺炎患者中的应用效果,重点分析对血气及神经功能指标的影响。方法:选取2023年1月~12月本院收治的脑梗死合并坠积性肺炎患者78例,采用随机数字表法分为对照组和观察组,各39例。对照组采用常规治疗,观察组在对照组基础上联合呼吸振荡排痰系统治疗,对比两组血气指标、血液流变指标、神经功能缺损指标、肺部啰音消失时间、平均住院时间及安全性指标。结果:干预7d后,观察组动脉血二氧化碳分压水平低于对照组,观察组血氧分压、pH、动脉血氧分压与吸入氧浓度比值水平高于对照组,血液流变指标全血黏度、血浆黏度、低切黏度/高切黏度、血小板聚集率水平均低于对照组,脑血流量大于对照组,神经功能缺损评分低于对照组,格拉斯哥昏迷量表评分高于对照组,各临床进程时间均短于对照组(P<0.05)。结论:呼吸振荡排痰系统能更有效地改善脑梗死合并坠积性肺炎患者的氧合状态,促进二氧化碳排出,有助于患者神经功能的恢复,缩短临床病程,效果显著。 Objective:To explore the effect of respiratory oscillating sputum removal system in patients with cerebral infarction combined with hypostatic pneumonia,and to analyze its impact on blood gas and neurologic function indicators.Methods:From January to December 2023,78 patients with cerebral infarction complicated with hypostatic pneumonia admitted to our hospital were selected and randomly divided into control group and observation group,with 39 cases in each group.The control group was treated with routine treatment,and the observation group was treated with respiratory oscillating sputum removal system on the basis of the control group.The blood gas,hemorheological indicators,neurologic function deficit indicators,the time of disappearance of pulmonary rales,the average hospital stay and the safety indicators of the two groups were.Results:After 7 days of intervention,the level of PaCO_(2) in the observation group was lower than that in the control group,and the levels of PaO_(2) pH,PaO_(2)/FiO_(2) in the observation group were higher than those in the control group.The levels of hemorheological indicators WBV,PV,AI PAR were all lower than those in the control group,the cerebral blood flow was greater than that in the control group,the group of neurologic function deficit scores was lower than in the control group,the Glasgow coma scale scores were higher than those in the control group,and the durations of each clinical process were all shorter than those in the control(P<0.05).Conclusion:Respiratory oscillating sputum removal system can more effectively improve the oxygenation status of patients with cerebral infarction combined with hypostatic pneumonia promote the excretion of carbon dioxide,help the recovery of patientsʼneurologic function,shorten the clinical course,and the effect is significant.
