腺样体低温等离子射频消融术治疗儿童阻塞性睡眠呼吸暂停低通气综合征的临床效果

摘要 目的:探讨儿童阻塞性睡眠呼吸暂停低通气综合征(OSAHS)采用腺样体低温等离子射频消融术治疗的效果。方法:选取2022年5月~2025年1月本院收治的102例OSAHS患儿,采用随机数字表法分成观察组和对照组各51例。对照组应用扁桃体联合腺样体切除术,观察组应用腺样体低温等离子射频消融术治疗,比较两组围手术期指标、睡眠质量、呼吸功能、生活质量、并发症。结果:观察组术中出血量低于对照组(P<0.05),手术时间、疼痛持续时间、住院时间短于对照组(P<0.05);观察组夜晚睡眠时长、每日睡眠时长高于对照组(P<0.05),夜醒次数、白日睡眠次数低于对照组(P<0.05);术后观察组术后呼吸暂停低通气指数、氧减指数低于对照组(P<0.05),最低夜间血氧饱和度高于对照组(P<0.05);术后观察组儿童OSAHS特异性生活质量调查量表(OSA-18)评分低于对照组(P<0.05);观察组并发症发生率低于对照组(P<0.05)。结论:腺样体低温等离子射频消融术应用于OSAHS患儿治疗中,能够优化围手术期指标,提高睡眠质量、呼吸功能及生活质量,减少并发症发生。 Objective:To investigate the efficacy of adenoid low-temperature plasma radiofrequency ablation in the treatment of obstructive sleep apnea hypopnea syndrome(OSAHS)in children.Methods:102 children with OSAHS admitted to our hospital from May 2022 to January 2025 were selected and randomly divided into an observation group and a control group,with 51 cases in each group,using a random number table method.The control group was treated with tonsillectomy combined with adenoidectomy,while the observation group was treated with low-temperature plasma radiofrequency ablation of adenoids.The perioperative indicators,sleep quality,respiratory function,quality of life,and complications were compared between the two groups.Results:The intraoperative blood output of the observation group was lower than that of the control group(P<0.05),and the operation time,pain duration,and hospitalization time were shorter than those of the control group(P<0.05);The observation group had higher nighttime sleep duration and daily sleep duration than the control group(P<0.05),and lower nighttime wake-up frequency and daytime sleep frequency than the control group(P<0.05);The postoperative apnea hypopnea index and oxygen depletion index in the observation group were lower than those in the control group(P<0.05),and the lowest nighttime oxygen saturation was higher than that in the control group(P<0.05);The postoperative observation group had lower scores on the OSAHS Specific Quality of Life Survey(OSA-18)compared to the control group(P<0.05);The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion:The application of adenoid low-temperature plasma radiofrequency ablation in the treatment of OSAHS in children can optimize perioperative indicators,improve sleep quality,respiratory function,and quality of life,and reduce the occurrence of complications.
