摘要
目的:探讨乳腺癌的MRI特征、外泌体浓度及肿瘤相关基因表达三者之间的关联,并评估这些生物标志物对患者预后的影响。方法:本研究收集了100名未经治疗的乳腺癌女性患者的数据。应用高分辨率MRI技术评估肿瘤的大小、形状及边缘特征,通过差速离心法与超速离心法从血清样本中提取外泌体,并利用实时定量聚合酶链反应(qPCR)技术分析乳腺癌易感基因1(BRCA1)和人表皮生长因子受体2(HER2)基因的表达水平。使用方差分析(ANOVA)和Pearson相关系数评估MRI表现、外泌体表达与基因表达三者之间的关系。结果:MRI特征分析显示,肿瘤的形状和边缘特征与乳腺癌的分子亚型密切相关。外泌体浓度与BRCA1和HER2基因表达之间存在显著正相关(Pearson相关系数为0.65和0.72,P<0.001)。多变量分析表明,肿瘤的MRI表现、外泌体浓度及基因表达水平是影响乳腺癌患者预后的独立因素。结论:乳腺癌的MRI表现、外泌体表达与肿瘤恶性增殖基因表达之间存在显著关联,这些生物标志物对患者的预后有显著影响。外泌体浓度及特定基因的高表达可能反映了肿瘤的生物学活性和恶性程度,这为乳腺癌的早期诊断和治疗提供了新的策略。
Purpose:To investigate the relationships among MRfeatures,serum exosome concentration,and expression of tumor-associated genes in breast cancer,and to evaluate their prognostics value.Methods:Data from 100 treatment-naive female breast cancer patients were analyzed.High-resolution MRwas used to assess tumor size,shape,and margin characteristics.Exosomes were isolated from serum samples by differential centrifugation combined with ultracentrifugation.Expression levels of the breast cancer susceptibility gene 1(BRCA1)and the human epidermal growth factor receptor 2(HER2)gene were quantified using real-time quantitative polymerase chain reaction(qPCR).ANOVA and Pearson correlation analysis were applied to examine the associations among MRfindings,exosome concentration,and gene expression.Results:MRfeatures,especially tumor shape and margin characteristics of,showed a close correlation with molecular subtypes of breast cancer.Exosome concentration was significantly positively correlated with the expression of BRCAl and HER2 genes(Pearson's r—0.65 and 0.72,respectively,P<0.001).Multivariate analysis revealed that MRfeatures,exosome concentration,and gene expression levels were independent prognostic factors for breast cancer patients.Conclusion:Significant associations exist among MRl features,exosome concentration,and expression of tumor-related genes in breast cancer.These biomarkers demonstrate considerable prognostics relevance.Elevated exosome concentration and high expression of specific genes may reflect tumor biological activity and aggressiveness,offering potential new strategies for early diagnosis and treatment of breast cancer.
