摘要
目的:讨论以护理为核心的早期综合康复模式对自闭症患儿多维功能的改善效果。方法:选取2022年11月1日—2024年12月31日福州市第一总医院儿童专科院区康复医学科门诊及病房收治的54例自闭症患儿作为研究对象。根据随机数表法将患儿分为观察组和对照组,每组各27例。对照组采用常规的护理方式,观察组在对照组护理模式基础上加用早期综合康复护理(早期行为干预护理、早期感统刺激护理、语言与沟通能力护理、情绪调节与睡眠护理、日常生活能力早期训练以及家庭早期护理指导)。分组护理3个月后,比较两组护理干预后简单反应时间、注意力、信息处理速度、自闭症行为检查量表(ABC)评分、心理教育评估第3版(PEP-3)评分、匹兹堡睡眠质量指数(PSQI)评分以及护理满意度。结果:干预后两组简单反应时间较干预前均明显缩短,信息处理速度明显提升,且观察组优于对照组,差异有统计学意义(P<0.05);干预后两组ABC评分及PEP-3评分较干预前均明显下降,且观察组各评分低于对照组,差异有统计学意义(P<0.05);干预后观察组PSQI6个维度评分均明显低于对照组,差异有统计学意义(P<0.05);观察组护理满意度明显高于对照组,差异有统计学意义(P<0.05)。结论:以护理为核心的早期综合康复模式可有效改善自闭症患儿的反应速度、注意力、信息处理能力,提升行为与认知功能,改善睡眠质量。
Objective:Discuss the improvement effect of the early comprehensive rehabilitation model centered on nursing on the multi-dimensional functions of children with autism.Method:A total of 54 children with autism spectrum disorder treated at the outpatient clinics and inpatient wards of the Department of Rehabilitation Medicine,Pediatric Specialty Campus of Fuzhou First General Hospital between November 1,2022 and December 31,2024 were enrolled in this study.The participants were randomly assigned,using a random number table,to an observation group or a control group,with 27 children in each group.The control group received routine nursing care,whereas the observation group received early comprehensive rehabilitation nursing in addition to routine care.This integrated nursing model comprised early behavioral intervention nursing,early sensory integration stimulation nursing,nursing focused on language and communication skills,emotional regulation and sleep management nursing,early training in activities of daily living,and early family-centered nursing guidance.After a 3-month nursing intervention,the two groups were compared with respect to simple reaction time,attention,information processing speed,scores on the Autism Behavior Checklist(ABC),the Psychoeducational Profile-Third Edition(PEP-3),the Pittsburgh Sleep Quality Index(PSQI),and overall nursing satisfaction.Result:After the intervention,simple reaction time was significantly shortened and information processing speed was markedly improved in both groups compared with baseline,with the observation group demonstrating superior improvements relative to the control group;these differences were statistically significant(P<0.05).The ABC and PEP-3 scores in both groups decreased significantly after the intervention,and all post-intervention scores in the observation group were significantly lower than those in the control group(P<0.05).In addition,the mean scores across all six PSQI dimensions were significantly lower in the observation group than in the control group following the intervention(P<0.05).Nursing satisfaction was also significantly higher in the observation group compared with the control group(P<0.05).Conclusion:The early comprehensive rehabilitation model centered on nursing can effectively improve the reaction speed,attention,and information processing ability of children with autism,enhance behavioral and cognitive functions,and improve sleep quality.
