摘要
目的通过对我院门诊西药房2024年登记在册的不合理处方干预情况进行回顾性分析,探讨处方干预在提升合理用药与规范处方管理方面的作用,为临床药学工作的持续改进提供循证依据。方法依据《处方管理办法》、药品说明书等标准,对不合理处方进行规范性及合理性审核,并对其中存在问题进行分类、统计与回顾性分析。结果在1932张被干预的不合理处方中,不规范处方占53.52%,用药不适宜处方占46.48%。通过药师的有效干预,所有不合理处方均得到及时修改,显著提升了患者用药的安全性,促进了临床合理用药的落实。结论我院当前处方干预过程中仍存在部分系统性与流程性障碍,影响药师审方效率与准确性。建议通过建立前置审方系统、完善处方干预制度、加强院内合理用药培训等多维度措施,从系统与人员两方面协同优化,进一步提升处方质量与合理用药水平。
Objective To retrospectively analyze the interventions on irrational prescriptions registered in the outpatient pharmacy of our hospital in 2024,and to explore the role of prescription intervention in promoting rational drug use and standardizing prescription management,so as to provide evidence-based support for the continuous improvement of clinical pharmacy practice.Methods Based on the“Prescription Management Measures”,drug package inserts,and related clinical guidelines,irrational prescriptions were reviewed for compliance and appropriateness.Identified issues were classified,statistically analyzed,and retrospectively examined.Results A total of 1932 irrational prescriptions were intervened.Among them,non-standard prescriptions accounted for 53.52%,while inappropriate medication prescriptions accounted for 46.48%.Through timely intervention by pharmacists,all irrational prescriptions were corrected,significantly improving medication safety and facilitating the implementation of rational drug use in clinical practice.Conclusion Systemic and procedural barriers still exist in the current prescription intervention process in our hospital,which affects the efficiency and accuracy of prescription review by pharmacists.It is recommended to adopt multidimensional measures,such as establishing a preprescription review system,improving the prescription intervention mechanism,and strengthening in-hospital training on rational drug use,so as to collaboratively optimize both systems and personnel,thereby further enhancing prescription quality and rational drug use.
