摘要
目的:探讨多学科协作诊疗模式下,开展药学服务在儿童全身型幼年特发性关节炎合并巨噬细胞活化综合征治疗中的作用,分析药学服务通过协助临床药物治疗方案的制定、药物可及性的提高、药学监护及药物治疗管理的提供,提升儿童全身型幼年特发性关节炎合并巨噬细胞活化综合征治疗的安全性和有效性。方法:以我院1例儿童全身型幼年特发性关节炎合并巨噬细胞活化综合征的多学科协作诊疗经过为例,梳理药学服务在该患儿治疗过程中的价值。结果:通过多学科会诊为该患儿明确诊断及治疗方案,经过临床药师整合循证证据协助临床制定药物治疗方案、优化药物供应链及药学部内部沟通提高药物可及性、强化用药风险管控提供药学监护及药物治疗管理等手段,有效解决了该患儿全身型幼年特发性关节炎合并巨噬细胞活化综合征在治疗过程中的药物治疗相关问题。结论:通过药学服务更好地保障了患者的用药安全。药学服务是多学科协作诊疗模式中重要的组成部分。
Objective:To explore the role of pharmaceutical service in the treatment of children with systemic juvenile idiopathic arthritis combined with macrophage activation syndrome under a multidisciplinary collaborative diagnosis and treatment,and analyze the role of pharmaceutical service in assisting in the development of clinical drug treatment plans,improving drug accessibility,providing pharmaceutical monitoring and drug treatment management,so as to enhance the safety and effectiveness of the treatment of children with systemic juvenile idiopathic arthritis complicated by macrophage activation syndrome.Methods:Taking the multidisciplinary collaborative diagnosis and treatment process of a child with systemic juvenile idiopathic arthritis complicated with macrophage activation syndrome in our hospital as an example,the value of pharmaceutical service in the treatment process of the child is summarized.Results:Through multidisciplinary consultation,the diagnosis and treatment plan for the child were clarified.Clinical pharmacists integrated evidencebased support to assist the development of drug treatment plans,optimized drug supply chains and internal communication within the pharmacy department to improve drug accessibility,strengthened drug risk control,and provided pharmaceutical supervision and drug treatment management.These measures effectively resolved the problems related to drug treatment in the treatment process of the child with systemic juvenile idiopathic arthritis complicated with macrophage activation syndrome.Conclusion:Through pharmaceutical service,the medication safety of patients has been better achieved.Pharmaceutical service is an important component of the multidisciplinary collaborative diagnosis and treatment model.
