摘要
目的:探究卵巢-附件影像报告和数据系统(O-RADS)超声评分联合血清癌抗原125(CA-125)、糖原199(CA19-9)检测在评估卵巢肿瘤良恶性中的临床价值。方法:选取2020年9月-2025年2月宣城市人民医院收治的111例接受手术治疗的卵巢肿瘤患者,经术后病理学检查分为恶性组(n=30例)和良性组(n=81例),检查显示共计124个包块,其中恶性包块37个,良性包块87个。收集患者的一般资料、血清学指标和卵巢O-RADS超声分类结果。分析卵巢肿瘤良恶性的影响因素并评估研究指标对患者卵巢恶性肿瘤的预测价值。结果:恶性组肿瘤最大直径≥10 cm、内部实性成分≥3 cm、内壁情况不光滑、血流评分1~2分的占比均高于良性组(P<0.05);经卵巢O-RADS超声评分得出卵巢恶性肿瘤包块32个,良性肿瘤包块92个,其中O-RADS 2类43个,3类49个,4类18个,5类14个。以组织病理学检查为金标准,卵巢O-RADS超声评分检测卵巢肿瘤良恶性的准确率89.52%,Kappa值为0.739;恶性组患者的血清CA-125阳性、血清CA19-9阳性的占比均高于良性组(P<0.05);血清CA-125、CA19-9阳性均为卵巢恶性肿瘤的危险因素(P<0.05);血清CA-125+CA19-9检测、卵巢O-RADS超声评分及超声+血清指标预测卵巢恶性肿瘤的AUC分别为0.743、0.890、0.916,预测效能均较高(P<0.05)。结论:血清CA-125、CA19-9水平与卵巢肿瘤良恶性密切相关,通过卵巢O-RADS超声评分与血清CA-125、CA19-9水平联合检测有助于临床对卵巢肿瘤的良恶性进行诊断。
Objective:To explore the clinical value of ovarian-adnexal reporting and data system(O-RADS)ultrasound score combined with serum carcinoma antigen 125(CA-125)and carbohydrate antigen 199(CA19-9)detection on evaluating benign and malignant ovarian tumors.Methods:From September 2020to February 2025,111 patients with ovarian tumors who underwent surgical treatment in Xuancheng People's Hospital were divided into malignant group(n=30 cases)and benign group(n=81 cases)by postoperative pathological examination,a total of 124 masses were found,including 37 malignant masses and 87 benign masses.The general data,serological indexes and O-RADS ultrasound classification results of the patients were collected.To analyze the influencing factors of benign and malignant ovarian tumors and evaluate the predictive value of research indicators for ovarian malignant tumors in patients.Results:The proportions of tumor maximum diameter≥10 cm,internal solid component≥3 cm,unsmooth inner wall and blood flow score of 1~2 points in malignant group were higher than those in benign group(P<0.05).According to the ovarian O-RADS ultrasound score,there were 32 malignant ovarian tumor masses and 92 benign tumor masses,including 43 cases of O-RADS 2,49 cases of O-RADS 3,18 cases of O-RADS 4 and 14 cases of O-RADS 5.The accuracy of ovarian O-RADS ultrasound score in detecting benign and malignant ovarian tumors was 89.52%,and the Kappa value was 0.739.The proportions of positive serum CA-125 and positive serum CA19-9 were higher in malignant group than those in benign group(P<0.05).Positive serum CA-125 and positive CA19-9 were risk factors of malignant ovarian tumors(P<0.05).The AUCof serum CA-125+CA19-9 detection,ovarian O-RADS ultrasound score and ultrasound+serum indicators in predicting malignant ovarian tumors were 0.743,0.890 and 0.916,and the sensitivities were 63.33%,85.19%and 96.67%,and the specificities were 85.19%,91.36%and 75.31%respectively,with high predictive efficiency(P<0.05).Conclusion:Serum CA-125 and CA19-9 levels are related to benign and malignant ovarian tumors in patients.The combined detection with ovarian O-RADS ultrasound score and serum CA-125 and CA19-9 levels is helpful for clinical diagnosis of benign and malignant ovarian tumors in patients.
