HIV抗病毒靶点研究及潜伏病毒库根除治疗策略研究进展

摘要 目前常用的针对人类免疫缺陷病毒(human immunodeficiency virus,HIV)生命周期不同阶段的抗反转录病毒药物在临床治疗HIV感染过程中取得了极大进展。然而,由于病毒库长期处于潜伏状态并在少数感染细胞中持续存在,通过抗反转录病毒治疗(antiretroviral therapy,ART)无法使其彻底清除;治疗过程中也存在一些病毒耐药突变和药物不良反应等问题,而获得性免疫缺陷综合征(acquired immunodeficiency syndrome,AIDS)的治愈研究和疫苗研发仍处于实验阶段。因此,在缺乏保护性疫苗和强化ART无法根除病毒的情况下,根除潜伏病毒库是目前治疗HIV感染的迫切需求。本文主要从HIV的药物作用靶点及机制、ART治疗效果影响因素和潜伏病毒库的根除策略3方面对HIV抗病毒靶点及潜伏病毒库根除治疗策略进行讨论,以期为进一步寻找新的药物作用模式和靶点提供参考。 The common antiretroviral drugs targeting different stages of the human immunodeficiency virus(HIV)lifecycle have made great progress in clinical treatment of HIV infection.However,due to the long-term incubation of the virus library and its persistent presence in a few infected cells,it cannot be completely eliminated through antiretroviral therapy(ART);There are also some drug-resistant mutations and adverse drug reactions in the treatment process,while the cure research and vaccine development of AIDS are still in the experimental stage.Therefore,in the absence of protective vaccines and enhanced efficient antiretroviral therapy that cannot eradicate the virus,eradicating latent reservoirs is currently an urgent need for the treatment of HIV infection.This review mainly discussed HIV antiviral targets and strategies for eradicating latent virus pools from three aspects:drug targets and mechanisms of HIV,factors affecting ART treatment efficacy,and eradication strategies for latent virus pools.The aim was to providere fer ence for further searching for new drug action modes and targets.
