基于针灸研究探讨内热针治疗中风后偏瘫潜在机制与跨病种转化证据

Exploration of the Potential Mechanisms of Internal Heat Needle in Post-Stroke Hemiplegia and Translational Evidence Across Diseases Based on Acupuncture

摘要 中风后偏瘫是脑卒中致残的核心并发症,传统针灸虽具疗效,但对慢性痉挛性偏瘫的干预效果有限。内热针疗法(intradermal heat needle therapy,IHNT)通过融合针刺与热疗(40~60℃),展现出独特优势。该文系统整合针灸与电针在中风偏瘫中的机制研究,包括调控炎症反应、氧化应激、细胞凋亡及血管新生等,结合内热针在其他疾病(如骨关节炎、腰椎间盘突出症)中的热效应证据,提出治疗中风偏瘫的跨病种转化逻辑。研究表明,内热针的热效应可强化传统针灸的共性通路[如核因子-κB(NF-κB)抑制效率提升、血管内皮生长因子(VEGF)表达量增加],并通过增强组织穿透性、促进能量代谢等独特机制,形成“热-针协同”的多靶点干预网络。尽管直接以内热针为干预手段的中风研究尚少,但病理共性及热效应优势为其临床转化提供了理论合理性。未来需通过专属性实验验证疗效,并借助人工智能优化操作参数,推动内热针在脑卒中康复中的规范化应用。 Post-stroke hemiplegia,a core disabling complication of cerebral stroke,presents significant challenges in chronic spasticity management.While traditional acupuncture demonstrates therapeutic efficacy,its limitations in modulating chronic spastic hemiplegia necessitate innovative approaches.Intradermal heat needle therapy(IHNT),integrating acupuncture with thermotherapy(40-60℃),exhibits unique advantages.This study systematically synthesizes mechanistic evidence of acupuncture and electroacupuncture in post-stroke hemiplegia,e.g.,neuroinhibition,oxidative stress mitigation,and apoptosis regulation,and integrates thermotherapeutic evidence from IHNT applications in other pathologies(e.g.,osteoarthritis and lumbar disc herniation),proposing a cross-disease translational rationale for IHNT in stroke rehabilitation.Findings indicate that IHNT's thermal effects amplify shared pathways of traditional acupuncture,such as enhanced nuclear factor-κB(NF-κB)inhibition and upregulated vascular endothelial growth factor(VEGF)expression,while establishing a“heat-needle synergy”network through unique mechanisms including improved tissue penetration and energy metabolism modulation.Although direct clinical studies of IHNT in stroke remain limited,pathophysiological commonalities and thermotherapeutic superiority provide theoretical justification for clinical translation.Future research should validate efficacy through disease-specific trials and optimize operational parameters using artificial intelligence to promote the standardized application of IHNT in stroke rehabilitation.
