摘要
颅内复杂动脉瘤为难治性脑血管病变,一般定义为因动脉瘤形态、大小、瘤颈结构、载瘤动脉与分支血管关系等因素导致常规显微外科夹闭或血管内治疗手段难以安全、有效实施的颅内动脉瘤。近年来随着显微手术和术中监测技术的进步,脑血管旁路移植术在颅内复杂动脉瘤中的应用日趋成熟。该文综述了颅内复杂动脉瘤行脑血管旁路移植术的分类、适应证、供体及受体血管选择、术前评估、术中监测及其安全性和有效性,旨在为临床医师针对颅内复杂动脉瘤制定个体化治疗策略提供参考。
Complex intracranial aneurysms are refractory cerebrovascular lesions.In the context of microsurgical clipping or endovascular treatment,complex intracranial aneurysms are defined as intracranial aneurysms that cannot be safely and effectively managed using conventional methods due to factors such as morphology,size,aneurysm neck structure,and the intricate relationship between the parent artery and its branch vessels.In recent years,with advancements in microsurgical techniques and intraoperative monitoring,the application of cerebral revascularization for the treatment of complex intracranial aneurysms has become increasingly mature,demonstrating overall safety and efficacy.This article reviewed the classification of bypass methods,indications,selection of donor and recipient vessels,preoperative assessment,intraoperative monitoring,and clinical effectiveness aiming to provide a comprehensive reference for clinicians to formulate individualized treatment strategies for intracranial complex aneurysms.
