2012—2023年重庆市早发脑卒中发病率与死亡率变化趋势

Trends of incidence and mortality of early-onset stroke in Chongqing City from 2012 to 2023

摘要 分析2012—2023年重庆市早发脑卒中发病率与死亡率变化趋势。早发脑卒中发病死亡患者数据来自重庆市心脑血管事件报告系统,收集计算发病率、标化发病率、死亡率、标化死亡率等资料。发病率和死亡率的趋势变化采用Joinpoint回归分析,并计算平均年度变化百分比(AAPC)。结果显示,重庆市早发脑卒中发病率与标化发病率由2012年的9.45/10万、7.08/10万上升至2023年的21.94/10万、21.64/10万,AAPC分别为4.84%、6.97%,变化趋势均有统计学意义(均P<0.05)。历年男性早发脑卒中发病率和标化发病率均高于女性(均P<0.01)。2012—2014年和2020—2023年,城市早发脑卒中发病率高于农村(均P<0.05),2017年农村早发脑卒中发病率高于城市(χ^(2)=5.951,P=0.015)。重庆市早发脑卒中死亡率与标化死亡率2012年分别为5.02/10万和3.77/10万,2023年分别为2.55/10万和2.52/10万,AAPC分别为-6.62%、-4.77%(均P<0.001)。历年男性早发脑卒中死亡率均高于女性,差异均有统计学意义(均P<0.05)。除2013年和2020年,农村早发脑卒中死亡率均高于城市(均P<0.05)。综上,2012—2023年重庆市早发脑卒中发病率和死亡率均较低,发病率呈现上升而死亡率呈现下降的趋势,男性和农村地区是早发脑卒中防治的重点。 This study analyzed the incidence and mortality of early-onset stroke and its trend in Chongqing City from 2012 to 2023.The cases of early-onset stroke were obtained from the Chongqing reporting system of Cardiovascular and Cerebrovascular Events.Indicators such as incidence,age-standardized incidence rate(ASIR),mortality,and age-standardized mortality rate(ASMR)were calculated.The trend of the incidence and mortality was analyzed using Joinpoint regression to calculate the average annual percentage change(AAPC).The results showed that the incidence and ASIR of early-onset stroke increased from 9.45 per 100000 and 7.08 per 100000 in 2012 to 21.94 per 100000 and 21.64 per 100000 in 2023,with the AAPC of 4.84%and 6.97%respectively,and both changing trends were statistically significant(both P<0.05).The incidence in males was higher than that in females in all years(all P<0.01).From 2012 to 2014 and 2020 to 2023,the incidence of stroke in urban areas was higher than that in rural areas(P<0.05),while in 2017,the incidence in rural areas was higher than that in urban areas(χ^(2)=5.951,P=0.015).In Chongqing,the mortality and ASMR of early-onset stroke were 5.02 per 100000 and 3.77 per 100000 in 2012,and 2.55 per 100000 and 2.52 per 100000 in 2023.Its AAPC was-6.62%and-4.77%,with statistically significant downward trends in both cases(both P<0.001).Over the years,the mortality of early-onset stroke in males was consistently higher than that in females(all P<0.05).Except for 2013 and 2020,the mortality of early-onset stroke in rural areas was consistently higher than in urban areas,with statistically significant differences(all P<0.05).In summary,the incidence of early-onset stroke in Chongqing is relatively low but shows a rapid upward trend,while its mortality is declining from 2012 to 2023.Males and rural areas are the key focus for the prevention and control of early-onset stroke.
