摘要
目的探讨丁香五套丸对脓毒症大鼠的脏器的保护作用。方法50只大鼠随机分5组,对照组、模型组、丁香五套丸低剂量组(1.755 g·kg^(-1)·d^(-1))、丁香五套丸中剂量组(3.51 g·kg^(-1)·d^(-1))、丁香五套丸高剂量组(7.02 g·kg^(-1)·d^(-1))。采用盲肠结扎穿孔方法复制脓毒症模型,模型组和丁香五套丸干预组分别在造模后给予生理盐水和丁香五套丸灌胃,每天1次,连续7 d,记录每天大鼠的生存状态。在第7天取材,留取外周血检测各组大鼠Treg细胞,留取血清并采用生化仪和ELISA法,检测各组大鼠心功能、肝功能、肾功能、肠功能损伤指标。结果与对照组相比,模型组大鼠生存时间减少(6/14),心体比、胸腺指数、派式结节数量和结肠长度均有降低,脾指数增加,炎症因子肿瘤坏死因子α(tumor necrosis factor-α,TNF-α),白细胞介素6(interleukin-6,IL-6)、白细胞介素10(interleukin-10,IL-10)、白细胞介素-18(interleukin-18,IL-18)水平均显著增加,心功能指标肌酸激酶同工酶(CK-MB)、肌钙蛋白T(cTnT)水平显著增加,肾功能指标尿素(Urea)水平显著增加,肝功能指标总蛋白(total protein,TP)和球蛋白(globulin,GLB)水平均有增加,谷丙转氨酶(alanine aminotransferase,ALT)、白蛋白(albumin,ALB)和ALB/GLB比值均有下降,肠功能指标二胺氧化酶(diamine oxidase,DAO)水增加,肠分泌型免疫球蛋白A(secreted immunoglobulin A,sIgA)水平显著下降,Treg细胞比例、CD4细胞比例和CD4/CD8比值均显著增加,CD8细胞比例显著下降(P<0.05)。丁香五套丸干预后,大鼠生存时间明显增加(低剂量:1/10,中剂量:0/10,高剂量:2/10),心体比、胸腺指数、派式结节数量和结肠长度均有升高,脾指数降低,炎症因子TNF-α、IL-1β、IL-10、IL-18水平均显著下降,心功能指标CK-MB、cTn-T水平显著下降,肾功能指标Urea水平显著降低,肝功能指标ALB水平上升,GLB和ALB/GLB比值均有下降,肠功能指标DAO水平下降,sIgA水平显著增加,Treg细胞比例和CD8细胞比例下降、CD4细胞比例和CD4/CD8比值增加(P<0.05)。结论丁香五套丸能够改善脓毒症大鼠的器官功能损伤,其机制与抑制炎症因子表达、调节Treg细胞功能、改善机体免疫紊乱相关。其中,肠器官功能改善作用较为明显,而且,丁香五套丸中剂量组效果最佳。
Objective To investigate the protective effect of Dingxiang Wutao Pill(丁香五套丸)on the organs of sepsis rats.Methods Fifty rats were randomly divided into 5 groups:sham group,model group,Dingxiang Wuquan Pill low-dose group(1.755 g·kg^(-1)·d^(-1)),Dingxiang Wuquan Pill medium-dose group(3.51 g·kg^(-1)·d^(-1))and Dingxiang Wuquan Pill high-dose group(7.02 g·kg^(-1)·d^(-1)).The sepsis model was replicated by cecal ligation and perforation(CLP).The model group and Dingxiang Wutao Pill intervention groups were given the normal saline and Dingxiang Wutao Pill by instillation once a day for 7 days,respectively,and the survival status of the rats was recorded every day.On the 7th day,the peripheral blood and serum were collected to detect Treg cells in each group,and the indexes of cardiac function,liver function,renal function and intestinal function were detected by biochemical analyzer and Enzyme-Linked Immunosorbent Assay(ELISA).Results Compared with those of the control group,the survival time of the rats in the model group was decreased(6/14),the heart-body ratio,thymus index,the number of PI nodule and colon length were decreased,the spleen index was increased,the levels of inflammatory factors tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-10(IL-10)and interleukin-18(IL-18)were significantly increased,and the levels of cardiac function indexes,creatine kinase-MB(CK-MB)and cardiac Troponin-T(cTn-T)were significantly increased.Urea level as the renal function indicator increased significantly.The levels of total protein(TP)and globulin(GLB)as the liver function indicator increased.The levels of alanine aminotransferase(ALT),albumin(ALB)and ALB/GLB ratio decreased.The diamine oxidase(DAO)water as the intestinal function indicator increased.The secreted immunoglobulin A(sIgA)level decreased significantly.Treg cell ratio,CD4 cell ratio and CD4/CD8 ratio all increased significantly.CD8 cell ratio decreased significantly.After the intervention of Dingxiang Wutao Pill,the survival time of rats was significantly increased(low dose:1/10,medium dose:0/10,high dose:2/10),accompanied with increased cardio-body ratio,thymus index,number of pie nodule,colon length,ALB,sIgA,CD4 cell proportion,CD4/CD8 ratio and decreased spleen index,TNF-α,IL-1β,IL-10,IL-18,CK-MB,cTn-T,Urea,GLB,ALB/GLB ratio,DAO,Treg cell proportion and CD8 cell proportion.Conclusion Dingxiang Wutao Pill can improve the organ function injury of sepsis rats,and its mechanism is related to inhibiting the expression of inflammatory factors,regulating the function of Treg cells and improving the immune disorder.Among them,the improvement of intestinal organ function was more obvious,and the medium dose group of Dingxiang Wutao Pill had the best effect.
