摘要
目的分析活血通络方熏洗在创伤性胫骨骨折患者中的应用效果。方法选取2021年1月—2023年6月苏州市中西医结合医院收治的79例研究对象,均为创伤性胫骨骨折患者,将其分为对照组(39例,采用常规治疗+功能锻炼)和研究组(40例,采用常规治疗+功能锻炼+活血通络方熏洗),参考随机数字表法分配原则,两组均随访6个月。比较两组消肿、骨折愈合、止痛、完全负重时间,治疗前后美国特种外科医院(hospital for special surgery,HSS)评分、Lysholm评分、Harris评分、疼痛程度、肿胀程度、膝关节活动度(knee joint activity,ROM)、血清骨形态发生蛋白-2(bone morphogenetic protein-2,BMP-2)、转化生长因子-β1(transforming growth factor-β1,TGF-β1)、碱性成纤维细胞因子(basic fibroblast growth factor,bFGF)、骨代谢指标水平以及住院期间并发症情况。结果对照组、研究组比较,后者消肿、骨折愈合、止痛、完全负重时间更短;治疗后对照组、研究组HSS、Lysholm、Harris评分、血清BMP-2、TGF-β1、bFGF、总Ⅰ型前胶原氨基端肽(total typeⅠprocollagen amino terminal peptide,t-PⅠNP)、Ⅰ型前胶原羧基端(typeⅠprocollagen carboxyl terminal,PⅠCP)水平、膝关节ROM与治疗前相比,两组均呈现升高趋势,且后者均更高;治疗后对照组、研究组血清BALP、β-CTX水平、疼痛程度、肿胀程度与治疗前相比,两组均呈现下降趋势,且后者更低;对照组、研究组住院期间并发症总发生率分别为23.08%(9/39)、5.00%(2/40),且经比较,后者更低,差异有统计学意义(P<0.05)。结论创伤性胫骨骨折患者应用活血通络方熏洗,改善关节功能,缓解临床症状,调节成骨相关因子、骨代谢指标表达水平,进而缩短恢复进程,同时并发症少。
Objective To analyze the application effect of Huoxue Tongluo Decoction(活血通络方)fumigation in patients with traumatic tibial fracture.Methods A total of 79 patients with traumatic tibial fracture admitted to Suzhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2021 to June 2023,and according to the principle of random number table,they were divided into the control group(39 cases,routine treatment+functional exercise)and the research group(40 cases,routine treatment+functional exercise+Huoxue Tongluo Decoction fumigation).Both groups were followed up for 6 months.The time of detumescence,fracture healing,pain relief and full weight-bearing,the Hospital for Special Surgery(HSS)score,Lysholm score,Harris score,pain degree,swelling degree,knee joint activity(ROM),the serum levels of bone morphogenetic protein-2(BMP-2),transforming growth factor-β1(TGF-β1),basic fibroblast growth factor(bFGF),bone metabolism index before and after treatment and complications during hospitalization were compared between the two groups.Results Compared with those of the control group,the research group had shorter swelling,fracture healing,pain relief and full weight-bearing time.After treatment,the HSS,Lysholm,Harris scores,serum levels of BMP-2,TGF-β1,bFGF,total typeⅠprocollagen amino terminal peptide(t-PⅠNP),typeⅠprocollagen carboxyl terminal(PⅠCP)and knee joint ROM of the control group and the research group showed an increasing trend compared with those before treatment,and those of the research group were higher.After treatment,the serum levels of BALP andβ-CTX,pain degree and swelling degree of the control group and the research group showed a downward trend compared with those before treatment,and those of the research group was lower.The total incidence of complications during the hospitalization of the control group and the research group was 23.08%(9/39)and 5.00%(2/40),respectively,and there were differences(P<0.05).Conclusion The application of Huoxue Tongluo Decoction in patients with traumatic tibial fracture can improve joint function,relieve clinical symptoms and adjust the expression levels of osteogenesis-related factors and bone metabolism indexes,thus shortening the recovery process and reducing complications.
