摘要
目的探究补中益气汤对老年ICU机械通气患者获得性肌无力的临床治疗效果。方法选取在该院治疗的重症监护室获得性肌无力(intensive care unit-acquired weakness,ICU-AW)患者100例,纳入研究的受试者使用随机分组的方法,分为对照组50例及观察组50例。对照组仅接受常规西医治疗,而观察组在常规治疗的基础上,额外配合补中益气汤,两组疗程均为4周,于治疗前后对比两组患者英国医学研究理事会(medical research council,MRC)肌力评分、改良Brarthel指数(modified barthel index,MBI)、总住院时间、ICU住院时间、机械通气时间、免疫功能及并发症发生情况。结果治疗后,治疗组及观察组MRC评分较治疗组显著提高、MBI指数也较治疗前水平显著提高(P<0.05),同时观察组MRC评分及MBI指数水平提高方面也明显优于对照组(P<0.05);两组患者肿瘤坏死因子-α(tumor necrosis factor-alpha,TNF-α)、白细胞介素-6(interleukin-6,IL-6)、D-二聚体、纤维蛋白原水平均较治疗前显著降低(P<0.05),观察组显著低于对照组(P<0.05);观察组患者总住院时间、ICU住院时间、机械通气时间明显较对照组缩短(P<0.05);两组患者总并发症发生率差异无统计学意义(P>0.05),对照组压疮发生率明显高于观察组(P<0.05)。结论补中益气汤能显著提高老年ICU-AW患者的临床疗效,改善患者临床症状,提升患者肌力及生存质量,促进患者恢复自理能力。
Objective To investigate the clinical therapeutic effect of Buzhong Yiqi Decoction(补中益气汤)on acquired muscle weakness in elderly ICU patients undergoing mechanical ventilation.Methods A total of 100 patients with ICU-acquired muscle weakness(ICU-AW)treated in the hospital were divided into a control group(50 cases)and an observation group(50 cases)using randomisation.The control group only received conventional Western medical treatment,while the observation group was additionally treated with Buzhong Yiqi Decoction on the top of the conventional treatment,and the course of treatment for both groups was 4 weeks,and the two groups were compared before and after the treatment with respect to the Medical Research Council of the United Kingdom(MRC)muscle strength scores,modified Brarthel index(MBI),total hospitalization time,ICU hospitalization time,mechanical ventilation time,immune function and the occurrence of complications.Results After treatment,the MRC score and MBI index of both groups were higher than those before treatment(P<0.05),and the scores of the observation group were significantly higher than those of the control group(P<0.05).The levels of tumor necrosis factor-alpha(TNF-α),interleukin-6(IL-6),D-dimer and fibrinogen aqueous mean of both groups were significantly lower than those before treatment(P<0.05),the results of the observation group were significantly lower compared to those of the control group(P<0.05).The total hospitalization time of patients in the observation group,ICU hospitalization time and mechanical ventilation time were significantly shorter in the observation group compared to those in the control group(P<0.05).The incidence rate of total complications did not exhibit a statistically significant difference between the two groups(P>0.05),and the control group showed a significantly higher incidence rate of pressure sores compared to the observation group(P<0.05).Conclusion Buzhong Yiqi Decoction can significantly improve the clinical efficacy and symptoms,enhance the muscle strength and quality of survival and promote the recovery of self-care ability of the elderly ICU-AW patients.
