摘要
目的研究温阳化瘀消癥汤联合穴位针刺治疗子宫内膜异位症(endometriosis,EMT)的疗效,探究对患者子宫动脉血流和血清转化生长因子-β(transforming growth factor-β,TGF-β)、白细胞介素-17(interleukin-17,IL-17)水平的影响。方法收集2022年3月—2024年4月在该院接受治疗的EMT患者80例,根据不同的治疗方法,将80例EMT患者分为联合组48例(温阳化瘀消癥汤联合穴位针刺)和针刺组32例(穴位针刺),比较两组方法治疗效果、中医证候积分、子宫动脉血流动力学指标、血清细胞因子水平、疼痛视觉模拟量表(visual analogue scale,VAS)评分及不良反应发生率。结果治疗后,联合组的临床有效率高于针刺组(P<0.05);治疗后,两组主证经行腹痛、腰骶不适、非经期腹痛、性交痛及次症腰酸腿软积分均明显降低(P<0.05),且联合组低于针刺组(P<0.05);两组患者左右侧子宫动脉最大血流速度均增大,且联合组大于针刺组,搏动指数、阻力指数均降低(P<0.05),且联合组低于针刺组(P<0.05);两组血清转化生长因子-β、白细胞介素-17水平均明显降低,差异有统计学意义(P<0.05),且联合组低于针刺组(P<0.05);两组患者VAS评分均下降,且联合组低于针刺组(P<0.05);两组不良反应发生率差异无统计学意义(P>0.05)。结论温阳化瘀消癥汤联合穴位针刺治疗EMT可调节患者血清IL-17、TGF-β水平,从而减少EMT的病变,改善患者子宫血流,疗效良好,安全可靠。
Objective To study the efficacy of Wenyang Huayu Xiaozheng Decoction(温阳化瘀消癥汤)combined with acupoint acupuncture in the treatment of endometriosis(EMT),and to explore the influence on uterine artery blood flow and serum levels of transforming growth factor-β(TGF-β)and interleukin-17(IL-17).Methods Eighty patients with EMT who received treatment in the hospital were collected from March 2022 to April 2024.According to different treatment methods,The patients were divided into combined group(48 cases,Wenyang Huayu Xiaozheng Decoction combined with acupoint acupuncture)and acupuncture group(32 cases,acupoint acupuncture).The therapeutic effect,TCM syndromes scores,uterine artery hemodynamic indicators,serum cytokines,visual analogue scale(VAS)score and incidence of adverse reactions were compared between groups.Results After treatment,the clinical effective rate in the combined group was higher than that in the acupuncture group(P<0.05).After treatment,the scores of primary symptoms such as abdominal pain,lumbosacral discomfort,non-menstrual abdominal pain and sexual intercourse pain and secondary symptom of lumbar soreness and leg weakness in the two groups were significantly decreased(P<0.05),and the above scores were lower in the combined group than those in the acupuncture group(P<0.05).The maximum blood flow velocities of the left and right uterine arteries were enhanced in both groups,and the velocities in the combined group were greater(P<0.05),and the pulsatility index and resistance index were reduced(P<0.05),and the two indexes were lower in the combined group(P<0.05).The levels of serum transforming growth factor-βand interleukin-17 were significantly decreased in both groups(P<0.05),and the levels in the combined group were lower than those in the acupuncture group(P<0.05).The VAS score in the two groups was decreased,and the score in the combined group was lower than that in the acupuncture group(P<0.05).There was no significant difference in the incidence of adverse reactions between groups(P>0.05).Conclusion Wenyang Huayu Xiaozheng Decoction combined with acupoint acupuncture can regulate the levels of serum IL-17 and TGF-βin patients with EMT,thereby reducing the EMT and improving the uterine blood flow of patients.In addition,it has good safety and it is safe and reliable.
