摘要
探讨单发套扎器联合聚桂醇行内痔套扎术+注射术的治疗效果及降低术后感染的影响。选择2021年7月—2022年7月医院收治的经肠镜检查确诊Ⅱ-Ⅲ度内痔患者120例作为研究样本进行回顾性分析,根据手术方式分为A组(单发套扎器套扎术)、B组(聚桂醇泡沫硬化剂注射术)和C组(单发套扎器套扎术联合聚桂醇泡沫硬化剂注射术),各40例,分别记录观察三组术中出血量、疼痛程度、术后创面状态、术后感染、3个月随访内痔愈合及复发等情况。C组术中出血量较A组和B组明显增高(P<0.05),但整体仍处于安全范围内,未发生严重出血事件。C组的术后创口愈合时间显著短于A组和B组(P<0.05),创口水肿时间也明显缩短。C组术后感染发生率为0.00%,显著低于A组和B组(P<0.05)。在术后疼痛评分方面,C组的疼痛感较B组更为明显,差异具有统计学意义(P<0.05),这表明联合治疗在术后早期可能引发较强的不适感。术后3个月随访结果显示,C组的脱垂程度评分和生活质量评分均显著改善,且C组的脱垂程度评分显著低于A组和B组(P<0.05)。此外,C组患者的生活质量较B组明显提高(P<0.05)。单发套扎器联合聚桂醇泡沫硬化剂注射术治疗Ⅱ-Ⅲ度内痔及部分混合痔,具有简便、安全、有效的特点,能够显著改善患者的临床症状、加速创口愈合,并降低术后感染的发生率。尽管术中出血量较大,但仍在安全范围内,适合临床推广应用。
To investigate the therapeutic effect of single-use ligator combined with polidocanol in internal hemorrhoid ligation and injection therapy,and its impact on reducing postoperative infections,a retrospective analysis was conducted on 120 patients diagnosed with grade II~III internal hemorrhoids by colonoscopy between July 2021 and July 2022.The patients were divided into three groups according to the surgical method:group A(single-use ligator ligation),group B(polidocanol foam sclerotherapy),and group C(single-use ligator ligation combined with polidocanol foam sclerotherapy),with 40 patients in each group.Intraoperative blood loss,pain levels,postoperative wound status,infection rates,and hemorrhoid healing and recurrence during 3 months of follow-up were recorded and compared across the three groups.The intraoperative bleeding volume in group C was significantly higher than that in groups A and B(P<0.05),but it remained within a safe range,and no severe bleeding events occurred.The wound healing time in group C was significantly shorter than that in groups A and B(P<0.05),and the wound edema time was also notably reduced.The postoperative infection rate in group C was 0.00%,significantly lower than that in groups A and B(P<0.05).Regarding postoperative pain scores,group C experienced more severe pain than group B,with a statistically significant difference(P<0.05)indicating that the combined treatment may cause stronger discomfort in the early postoperative period.Follow-up at 3 months post-surgery showed that both the prolapse degree and quality of life scores in group C significantly improved,and the prolapse degree in group C was significantly lower than that in groups A and B(P<0.05).Additionally,group C showed a significant improvement in quality of life compared to group B(P<0.O5).The combination of single-shot ligator and polidocanol foam sclerotherapy for the treatment of grade II-III internal hemorrhoids and some mixed hemorrhoids is simple,safe,and effective.It can significantly improve clinical symptoms,accelerate wound healing,and reduce the incidence of postoperative infections.Although the intraoperative bleeding volume was relatively high,it remains within a safe range,making it suitable for clinical promotion and application.
