摘要
探讨地夸磷索钠滴眼液与透明质酸钠滴眼液治疗2型糖尿病干眼症的临床疗效对比。前瞻性选取2021年01月—2024年01月就诊于苏州中西医结合医院并且诊断为2型糖尿病干眼症的患者116例,随机分配为对照组(n=58,透明质酸钠滴眼液治疗)和研究组(n=58,地夸磷索钠滴眼液治疗)。对照组采用0.1%透明质酸钠滴眼液治疗,研究组采用3%地夸磷索滴眼液治疗。在治疗前、治疗后第14 d和第28 d,评估并比较干眼症疾病严重程度、泪膜破裂时间和Schirmer泪液分泌试验、角膜地形图检查、泪膜脂质层厚度、睑板腺质量及睑板腺脂质排出能力评分。治疗前,两组患者的疾病严重程度评分无差异(P>0.05);治疗14 d和28 d后,疾病严重程度评分相比治疗前明显下降,相比对照组,研究组的下降更显著(P<0.05);治疗14 d和28 d后,BUT时间延长、SIT长度增加,相比对照组,研究组的增加更明显(P<0.05);角膜地形图显示治疗后SAI和SRI都降低,相比对照组,研究组的降低更显著(P<0.05)。治疗前,两组患者的泪膜脂质层无差异(P>0.05),治疗14 d和28 d后,相比对照组,研究组的升高更明显(P<0.05)。治疗前,两组患者的睑板腺质量及睑板腺脂质排出能力评分均较高,无差异(P>0.05);治疗14 d和28 d后,相比对照组,研究组的下降更显著(P<0.05)。本研究结果提示,3%地夸磷索滴眼液能够减轻糖尿病干眼症患者的疾病程度,提高泪膜稳定性,以及改善睑板功能。
To compare the clinical efficacy between diquafosol sodium eye drops and sodium hyaluronate eye drops in the treatment of type 2 diabetic dry eye,a total of 116 patients diagnosed with type 2 diabetic dry eye who visited Suzhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2021 to January 2024 were prospectively selected and randomly assigned to a control group(n=58 treated with sodium hyaluronate eye drops)and a study group(n=58 treated with diquafosol sodium eye drops).The control group was treated with 0.1%sodium hyaluronate eye drops,and the study group was treated with 3%diquafosol eye drops.The severity of dry eye disease,tear film breakup time and Schirmer tear secretion test,corneal topography examination,tear film lipid layer thickness,meibomian gland quality and meibomian gland lipid excretion ability score were evaluated and compared before treatment,on the 14th day and 28th day after treatment.Before treatment,there was no difference in the disease severity score between the two groups(P>0.05).After 14 and 28 days of treatment,the disease severity score decreased significantly compared with before treatment,and the decrease in the study group was more significant than that in the control group(P<0.05).After 14 and 28 days of treatment,the BUT time was prolonged and the SIT length increased,and the increase in the study group was more significant than that in the control group(P<0.05).Corneal topography showed that both SAI and SRI increased after treatment,and the increase in the study group was more significant than that in the control group(P<0.05).Before treatment,there was no difference in the tear film lipid layer between the two groups(P>0.05).After 14 and 28 days of treatment,the increase in the study group was more significant than that in the control group(P<0.05).Before treatment,the meibomian gland mass and meibomian gland lipid excretion ability scores of the two groups were high,with no difference(P>0.05).After 14 and 28 days of treatment,the decrease in the study group was more significant than that in the control group(P<0.05).The results of this study suggest that 3%diquafosol eye drops can reduce the severity of the disease in patients with diabetic dry eye,improve tear film stability,and improve tarsal function.
