高流量湿化氧疗联合穴位敷贴治疗老年肺部感染临床观察

Clinical Observation on High-Flow Nasal Cannula Therapy Joined with Acupoint Application in Treating Pulmonary Infection in the Elderly

摘要 目的:观察高流量湿化氧疗联合穴位敷贴治疗老年肺部感染的临床效果。方法:选取112例老年肺部感染患者,通过Excel产生的随机数字将其分为对照组和试验组,每组56例。对照组在基础治疗的同时予高流量湿化氧疗,试验组在对照组基础上予穴位敷贴治疗。比较两组患者血清相关炎症因子[白细胞介素6(interleukin-6,IL-6)、IL-8、肿瘤坏死因子α(tumor necrosis factorα,TNF-α)、C反应蛋白(C-reaction protein,CRP)、降钙素原(procalcitonin,PCT)]水平、肺功能指标[最大肺活量(forced vital capacity,FVC)、第1秒时间肺活量/用力肺活量(first second forced vital capacity/tidal volume,FEV_(1)/FVC)、第一秒时间肺活量(forced expiratory volume in one second,FEV_(1))及深吸气量(inspiratory capacity,IC)]、中医症状评分及肺部感染程度。结果:与治疗前比较,两组患者治疗3、14天后,IL-6、IL-8、TNF-α、CRP、PCT水平均降低(P<0.05),且治疗14天以上指标降低更明显(P<0.05);与同期对照组治疗后比较,试验组以上指标降低程度均优于对照组(P<0.05)。对照组治疗14天中医症状评分均低于治疗3天及治疗前(P<0.05);与治疗前比较,试验组治疗3、14天中医症状评分均降低(P<0.05),治疗14天降低更明显(P<0.05);与同期对照组治疗后比较,试验组中医症状评分降低程度均优于对照组(P<0.05)。对照组治疗14天肺功能各项指标均高于治疗3天及治疗前(P<0.05);与治疗前比较,试验组治疗3、14天,肺功能各项指标均升高(P<0.05),治疗14天升高更明显(P<0.05);与同期对照组治疗后比较,试验组肺功能各项指标升高程度均优于对照组(P<0.05)。治疗14天,试验组患者肺部感染程度较对照组减轻,差异具有统计学意义(P<0.05)。结论:高流量湿化氧疗联合穴位敷贴法能够降低老年肺部感染患者血清炎性因子水平,提高肺功能,改善肺部感染程度。 Objective:To explore clinical effects of high-flow nasal cannula(HFNC)therapy and acupoint application in treating pulmonary infection in the elderly.Methods:All 112 elderly patients with pulmonary infection were chosen,and they were allocated to the control group and the experiment group by means of a randomization sequence generated in Excel,with 56 cases per group,respectively.The control group received HFNC therapy alongside the basic therapy,and the experiment group,whereas,was treated with acupoint application in addition to HFNC therapy and the basic therapy.To compare the levels of serum inflammatory factors containing IL-6,IL-8,TNF-α,CRP and PCT,pulmonary function parameters such as FVC,FEV_(1)/FVC,FEV_(1) and IC,TCM symptom scores and severity of pulmonary infection between the two groups.Results:A decrease in the levels of IL-6,IL-8,TNF-α,CRP and PCT was observed in the two groups after 3 and 14 days of the treatment compared with these before the treatment(P<0.05),and the reduction in the indexes was more evident after 14 days of the treatment(P<0.05);compared with the control group after the same treatment period,the experiment group demonstrated a greater reduction in all of the above indicators(P<0.05).On day 14 of treatment,TCM symptom scores of the control group were lower than these on day 3 of treatment and before the treatment(P<0.05);compared with before the treatment,TCM symptom scores of the experiment group were reduced at both 3 and 14 days of treatment(P<0.05),the decrease was more significant at 14 days of treatment(P<0.05);compared to the control group after the same treatment period,the experiment group demonstrated a greater reduction in TCM symptom scores(P<0.05).The indexes of lung function of the control group at 14 days of treatment were higher than these on day 3 of treatment and before treatment(P<0.05);compared with before the treatment,the indexes of lung function were elevated at both 3 and 14 days of treatment in the experiment group(P<0.05),the increase was more evident at 14 days of treatment(P<0.05);compared with the control group after the same treatment period,the improvements of the indexes of lung function of the experiment group were better than these of the control group(P<0.05).After 14 days of treatment,pulmonary infection in the experiment group was less severe than that in the control group,and the difference had statistical meaning(P<0.05).Conclusion:HFNC therapy and acupoint application could lower the levels of serum inflammatory factors,improve lung function and pulmonary infection in elderly patients with pulmonary infection.
