摘要
大黄是我国传统大宗药材之一,功效为泻下攻积,清热泻火,凉血解毒,逐瘀通经,利湿退黄,其在治疗便秘方面具有悠久的历史和广泛的应用。便秘是最常见的功能性胃肠疾病之一,发病率连年升高,严重危害机体健康。目前化学药虽然在短期内能够取得显著的成效,但长期使用却往往存在疗效逐渐减弱及不良反应的问题。现代药理研究表明,大黄可调节肠道菌群及代谢产物、肠道黏液分泌,以及离子通道、水通道蛋白、神经递质等的表达,促进肠道动力。大黄能够通过不同的炮制方式、给药途径及联合用药等,多靶点、多通路、多途径发挥对便秘的治疗和保护作用。从大黄的炮制、最佳药效、临床应用、药理作用与毒理方面对大黄治疗便秘的研究进展进行综述,以期为今后大黄的深入研究应用与治疗便秘相关药物的研发提供一定的参考价值。
Dahuang(Rhei Radix et Rhizoma)is one of the traditional bulk medicinal materials in China.It has a long history and wide application in the treatment of constipation because of its efficacy of purging,clearing heat and purging fire,cooling blood and detoxifying,removing blood stasis and dredging meridians,and removing dampness and jaundice.Constipation is one of the most common functional gastrointestinal diseases.The incidence of constipation is increasing year by year,seriously endangering the health of the body.At present,although the chemical drugs can achieve remarkable results in the short term,the long-term use often has the problems of gradually weakened efficacy and adverse reactions.Modern pharmacological studies have shown that Dahuang(Rhei Radix et Rhizoma)can regulate intestinal flora and metabolites,intestinal mucus secretion,as well as the expressions of ion channels,aquaporins,neurotransmitters,etc.,and promote the intestinal motility.Dahuang(Rhei Radix et Rhizoma)can play a role in the treatment and protection of constipation through different processing methods,administration routes and combined medication,multi-target,multi-channel and multi-channel.This article reviewed the research progress of Dahuang(Rhei Radix et Rhizoma)in the treatment of constipation from the aspects of processing,optimal efficacy,clinical application,pharmacological effects and toxicology,in order to provide some reference value for the further research and application of Dahuang(Rhei Radix et Rhizoma)and the development of drugs related to the treatment of constipation.
