摘要
目的分析腕踝针联合参芪通痹方治疗糖尿病周围神经病变(气阴两虚挟瘀证)的疗效和对神经传导的影响。方法以符合研究标准的气阴两虚挟瘀证糖尿病周围神经病变患者82例进行分析,纳入时间2021年12月—2023年12月,将患者随机分为两组。对照组41例,接受常规西药治疗;观察组41例,采用腕踝针疗法结合参芪通痹方进行治疗。对两组的治疗效果进行对比分析,观察中医症状积分变化、多伦多临床评分(Toronto clinical scoring system,TCSS)、血糖变化、神经传导速度变化及血清胱抑素C(Serum cystatin C,Cys-C)、胰岛素生长因子-1(Insulin-like growth factor-1,IGF-1)、C肽(C-peptide,C-P)水平。结果观察组有效率95.12%(39/41),高于对照组的73.17%(30/41),组间比较差异有统计学意义(P<0.05);治疗后观察组中医症状积分及TCSS评分、血糖指标水平均低于对照组(P<0.05);治疗后观察组运动神经传导速度(Motor nerve conduction velocity,MNCV)、感觉神经传导速度(Sensory nerve conduction velocity,SNCV)水平高于对照组(P<0.05);治疗后观察组Cys-C水平降低,IGF-1及C-P水平提高,优于对照组,组间比较差异有统计学意义(P<0.05)。结论对气阴两虚挟瘀证糖尿病周围神经病变患者开展腕踝针结合参芪通痹方治疗,可以显著缓解肢体麻木和疼痛等不适,有助于降低血糖,并调整Cys-C、IGF-1、C-P的水平,同时提升神经传导速度,对患者的康复过程具有正面影响。
Objective To analyze the therapeutic effect of wrist-ankle acupuncture combined with Shenqi Tongbi Prescription(参芪通痹方)on diabetes peripheral neuropathy with deficiency of both Qi and Yin deficiency with blood stasis syndrome and its influence on nerve conduction.Method This study selected 82 patients with diabetic peripheral neuropathy(deficiency of both Qi and Yin deficiency with blood stasis syndrome)who were treated in the hospital from December 2021 to December 2023 as the research subjects.Using the random number table method,the patients were randomly divided into two groups,with 41 patients in each group.The control group received the conventional Western medicine treatment,while the observation group was treated with wrist-ankle acupuncture combined with Shenqi Tongbi Prescription.The therapeutic effects of the two groups were compared and analyzed,observing the changes in TCM syndrome scores,Toronto Clinical Scoring System(TCSS),blood glucose levels,nerve conduction velocity and the levels of serum cystatin C(Cys-C),insulin-like growth factor-1(IGF-1)and C-peptide(C-P).Results The overall effectiveness of the observation group was 95.12%(39/41)and that of the control group was 73.17%(30/41).The overall effectiveness of the observation group was higher than that of the control group,and the difference between the groups was significant(P<0.05).After treatment,the traditional Chinese medicine symptom scores,TCSS scores and blood glucose index levels in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of motor nerve conduction velocity(MNCV)and sensory nerve conduction velocity(SNCV)in the observation group were higher than those in the control group(P<0.05).After treatment,the Cys-C level in the observation group decreased,while the levels of IGF-1 and C-P increased,both of which were better than those of the control group,and the difference between the groups was significant(P<0.05).Conclusion In the treatment of patients with deficiency of both Qi and Yin deficiency with blood stasis syndrome of diabetic peripheral neuropathy,wrist-ankle acupuncture combined with Shenqi Tongbi Prescription can significantly alleviate limb numbness and pain,help to reduce blood sugar,regulate the levels of Cys-C,IGF-1 and C-P,and improve the nerve conduction speed,which has a positive impact on the rehabilitation process of patients.
