摘要
目的观察ICU感染性休克患者开放式与限制性液体复苏策略在临床结局和并发症控制方面的效果差异。方法纳入2023年5月—2025年4月本院重症监护病房收治的感染性休克患者80例,按复苏策略分为对照组(开放式液体复苏)和观察组(限制性液体复苏),各40例。比较两组复苏期24h补液量、休克持续时间和ICU住院时间,治疗前及治疗3d后肝肾功能指标及序贯器官衰竭评估评分变化,并随访1个月记录多器官功能障碍综合征、肺水肿及再次液体复苏等并发症发生情况。结果与开放式液体复苏相比,限制性液体复苏在减少早期补液量、缩短休克持续时间和ICU住院时间方面具有优势,肝肾功能及病情评分改善更为明显,并发症总发生率更低。结论在规范化综合救治框架下实施限制性液体复苏策略,有助于优化ICU感染性休克患者的容量管理,促进器官功能恢复并降低并发症风险,具有推广价值。
Objective To observe the differences in clinical outcomes and complication control between open and restricted fluid resuscitation strategies in ICU septic shock patients.Method 80 patients with septic shock admitted to the intensive care unit of our hospital from May 2023 to April 2025 were included.They were divided into a control group(open fluid resuscitation)and an observation group(restricted fluid resuscitation)according to the resuscitation strategy,with 40 patients in each group.Compare the 24-hour fluid replacement volume,shock duration,and ICU stay between two groups during the resuscitation period,as well as changes in liver and kidney function indicators and sequential organ failure assessment scores before and after 3 days of treatment.Follow up for 1 month will be conducted to record the occurrence of complications such as multiple organ dysfunction syndrome,pulmonary edema,and fluid resuscitation.Result Compared with open fluid resuscitation,restricted fluid resuscitation has advantages in reducing early fluid replacement,shortening shock duration,and ICU hospitalization time.The improvement of liver and kidney function and disease score is more significant,and the overall incidence of complications is lower.Conclusion Implementing a restrictive fluid resuscitation strategy within a standardized comprehensive treatment framework can help optimize capacity management for ICU septic shock patients,promote organ function recovery,and reduce the risk of complications.It has promotional value.
