基于“伏邪理论”探讨火针治疗在糖尿病周围神经病变的意义

摘要 糖尿病周围神经病变作为糖尿病常见的并发症之一,是糖尿病下肢神经病变、血管病变的共同结果,其病理机制复杂,涉及神经损伤、微血管病变及代谢紊乱等多方面因素。本文以“伏邪理论”结合现代医学中“代谢记忆”的认知,长期高血糖状态下,代谢紊乱、微循环障碍等因素导致的病理产物在体内积聚,形成“伏邪”,这些“伏邪”潜伏于经络、脏腑之中,逐渐损伤神经组织,引发糖尿病周围神经病变的临床症状。因此火针点穴疗法作为一种传统中医疗法,通过其特殊选穴和治疗机制为临床应用追根溯源,温热刺激,能够激发经络之气,推动气血运行,驱逐潜伏于经络中的“伏邪”,改善糖尿病周围神经病变患者的局部血液循环和神经功能起到疏通经络,调和阴阳,行气活血的作用,具有重要的临床意义。 Diabetic peripheral neuropathy(DPN),as one of the common complications of diabetes,is the joint result of diabetic lower limb neuropathy and vascular disease,and its pathogenesis is complex,involving nerve damage,microangiopathy and metabolic disorders.Based on the theory of latent pathogens,in combination with the insights of"metabolic memory"in modern medicine,this paper holds that pathological products,caused by metabolic disorders,microcirculation disorders and other factors under long-term hyperglycemia,accumulate in the body,forming"latent pathogens".The"latent pathogens"lurk in the meridians and viscera,gradually damaging nerve tissue and causing clinical symptoms of DPN.As a healing method in traditional Chinese medicine,fire needle acupuncture can warmly stimulate qi in meridians,promote qi and blood circulation,expel the"latent pathogens"lurking in meridians.Therefore,the therapy improves the local blood circulation and nerve function in DPN patients,playing the role of dredging meridians,harmonizing yin and yang,and promoting qi and blood,achieving significant clinical efficacy.
