摘要
早期复发性流产具有“应期而堕”的特点,其临床特点与吴鞠通《温病条辨·保胎论二》中提及的“每殒胎必三月”一症类似。吴鞠通认为该病病位在肝,归纳其病机为“肝虚而热”。本文基于“肝虚而热”理论,探讨调节女性孕前及孕后机体阴阳状态对治疗复发性流产的必要性,并以吴鞠通“肝虚而热”理论为基础探讨阴虚阳亢型早期复发性流产的病因病机,并分析吴鞠通的组方思路,认为治疗该类早期复发性流产需以补益肝肾、滋阴潜阳为核心,总结出其“两情合用,滋水涵木以潜阳”“阴阳兼顾,补泻合宜以复衡”“预培其损,长服调养以保胎”的用方特点,综合调节女性机体阴阳状态,以期为中医药防治阴虚阳亢型早期复发性流产提供新思路。
Early recurrent spontaneous abortion(ERSA)has the characteristics of"abortion in response to periods",and its clinical characteristics are similar to Wu Jutong's case record of"Abortions periodically occur in March"in Detailed Analysis of Epidemic Warm Diseases:Baotai Lun II.He believes that the disease is located in the liver,and its pathogenesis features"heat due to liver deficiency".Based on this theory,the article discusses the necessity of regulating the yin-yang state of women before and after pregnancy for the treatment of recurrent spontaneous abortion,delving into the etiology and pathogenesis of ERSA with the syndrome of"heat due to liver deficiency",and analyzing Wu Jutong's ideas for formulating prescriptions in this respect.It is believed that the treatment of ERSA with the syndrome of"heat due to liver deficiency"should focus on nourishing the liver and kidney,nourishing yin for suppressing hyperactive yang.In addition,the paper tentatively summarizes the prescription characteristics in clinical treatment of ERSA.It is concluded that a comprehensive regulation of the yinyang state of pregnant women is expected for the prevention and treatment of ERSA with the syndrome of"heat due to liver deficiency"in traditional Chinese medicine.
