摘要
非HACEK革兰阴性菌感染性心内膜炎是一种罕见但日益受到重视的疾病,具有高发病率和高死亡率的特点。铜绿假单胞菌、粘质沙雷氏菌、肺炎克雷伯杆菌和大肠埃希菌是主要病原体。本文报道1例血培养阴性的心内膜炎患者,通过宏基因组二代测序(mNGS)明确感染病原体为产KPC、NDM肺炎克雷伯杆菌,通过精准的抗感染治疗,感染指标明显下降。
Non-HACEK Gram-negative bacterial endocarditis is a rare but increasingly recognized disease,characterized by high morbidity and mortality.Pseudomonas aeruginosa,Serratia marcescens,Klebsiella pneumoniae,and Escherichia coli are recognized as the main pathogens.This article reports a case of endocarditis with negative blood culture.Patient’s pathogen was identified as KPC and NDM-producing Klebsiella pneumoniae through metagenomic next-generation sequencing(mNGS).Through precise anti-infection treatments,the infection indicators decreased significantly.
