病毒性心肌炎相关信号通路及其中药干预的研究进展

摘要 病毒性心肌炎(VMC)是导致儿童与青少年发生心源性猝死的主要原因,由于其病理机制仍不明确,因此尚无有效的治疗方法。中医药在治疗病毒性心肌炎方面显示出显著疗效,能够有效缓解症状并改善预后。近年来,大量实验证据显示,中药复方、单味中药及中药有效成分通过作用于多个关键信号通路,如核因子κB、哺乳动物雷帕霉素靶蛋白(mTOR)、核因子红细胞系2相关因子(Nrf2)/血红素加氧酶-1(HO-1)、c-Jun氨基末端激酶(JNK)与转化生长因子-β_(1)(TGF-β_(1))等,在VMC中发挥抗炎、抗氧化应激、抗细胞凋亡等作用,从而抑制疾病进展并改善心功能。此外,中医药因其多途径、多靶点的治疗特点,具备良好的安全性,展现出广阔的临床应用前景。 Viral myocarditis(VMC)is a major cause of sudden cardiac death in children and adolescents.Owing to the still unclear pathological mechanisms,effective therapeutic approaches remain unavailable.Traditional Chinese medicine(TCM)has demonstrated significant efficacy in the treatment of VMC,effectively alleviating clinical symptoms and improving prognosis.In recent years,a substantial body of experimental evidence has shown that TCM compound formulas,single herbal medicines,and active components exert anti-inflammatory,antioxidative stress,and anti-apoptotic effects in VMC by acting on multiple key signaling pathways,including nuclear factor kappa B(NF-κB),mammalian target of rapamycin(mTOR),nuclear factor erythroid 2-related factor 2(Nrf2)/heme oxygenase-1(HO-1),c-Jun N-terminal kinase(JNK),and transforming growth factor-β_(1)(TGF-β_(1)).Through these mechanisms,disease progression is inhibited and cardiac function is improved.In addition,owing to its multi-pathway and multi-target therapeutic characteristics,TCM exhibits good safety and demonstrates broad prospects for clinical application.
