摘要
目的探讨家庭药师主导的优质药学服务在慢性病患者居家用药管理中的应用效果。方法选取医院2024年1月至2025年1月收治的慢性病已出院患者120例,按随机数字表法分为研究组和对照组,各60例。对照组患者接受常规出院用药指导,研究组患者接受家庭药师主导的优质药学服务。比较两组患者的合理用药认知度、用药安全性、用药依从性、满意度、病情控制情况。结果干预后,研究组患者的合理用药认知度评分为(8.10±1.87)分,显著高于对照组的(6.97±1.22)分(P<0.001);药品不良反应发生率显著低于对照组(5.00%比21.67%,P=0.007),用药总依从率、患者满意度、病情控制优良率均显著高于对照组(95.00%比81.67%,96.67%比85.00%,85.00%比55.00%,P<0.05)。结论家庭药师主导的优质药学服务能提升慢性病患者的合理用药认知度、居家用药安全性、用药依从性、满意度及病情控制效果,改善患者的健康状况。
Objective To investigate the application effect of high-quality pharmaceutical care led by family pharmacists in the home-based medication management for patients with chronic diseases.Methods A total of 120 patients with chronic diseases admitted to the hospital from January 2024 to January 2025 were selected and randomly divided into the study group and the control group by the random number table method,with 60 cases in each group.The patients in the control group received routine discharge medication guidance,while the patients in the study group received high-quality pharmaceutical care led by family pharmacists.The awareness of rational drug use,medication safety,medication adherence,satisfaction,and disease control between the two groups was compared.Results After the intervention,the cognitive score of rational drug use in the study group was(8.10±1.87)points,which was significantly higher than(6.97±1.22)points in the control group(P<0.001).The incidence of adverse drug reactions in the study group was significantly lower than that in the control group(5.00%vs.21.67%,P=0.007),while the overall medication adherence rate,patient satisfaction rate,and excellent disease control rate in the study group were significantly higher than those in the control group(95.00%vs.81.67%,96.67%vs.85.00%,85.00%vs.55.00%,P<0.05).Conclusion High-quality pharmaceutical care led by family pharmacists can enhance the awareness of rational drug use,home-based medication safety,medication adherence,satisfaction,and disease control effectiveness of patients with chronic disease,and improve their health status.
