基于超声特征诊断中国甲状腺影像报告和数据系统4A与4B类甲状腺结节的良恶性的价值分析

摘要 目的:探究超声特征对中国甲状腺影像报告和数据系统(C-TIRADS)4A与4B类甲状腺结节良恶性的诊断效能。方法:回顾性收集2022年1月—2025年7月南京市高淳人民医院收治的126例C-TIRADS 4A与4B类甲状腺结节患者的临床资料。以病理检查结果为“金标准”,将患者分为良性组和恶性组。比较两组临床资料,采用多因素Logistic回归分析甲状腺结节良恶性的影响因素。绘制受试者工作特征(ROC)曲线,以曲线下面积(AUC)评价超声特征对甲状腺结节良恶性的诊断效能。结果:126例患者中良性组55例(43.65%)、恶性组71例(56.35%)。两组回声、纵横比、边界、钙化、弹性评分、造影增强模式及弹性成像应变比有统计学差异(P<0.001)。Logistic回归分析结果显示,纵横比、弹性评分及弹性成像应变比是影响甲状腺结节良恶性的独立危险因素(P<0.05)。ROC曲线分析结果显示,纵横比、弹性评分及弹性成像应变比鉴别诊断甲状腺结节良恶性的AUC分别为0.802、0.757、0.741,联合诊断的AUC为0.891,敏感度均高于单独指标(P<0.05)。结论:纵横比、弹性评分及弹性成像应变比是C-TIRADS 4A与4B类甲状腺结节良恶性的危险因素,三项指标联合检测可提高甲状腺结节良恶性鉴别诊断敏感度,具有良好的诊断效能。 Objective:To explore the diagnostic efficiency of ultrasound features in benign and malignant thyroid nodules of Chinese version of thyroid imaging reporting and data system(C-TIRADS)Grades 4A and 4B.Methods:The clinical data of 126 patients with C-TIRADS 4A and 4B thyroid nodules admitted to Nanjing Gaochun People's Hospital from January 2022 to July 2025 were retrospectively collected.Taking the results of pathological examination as the"gold standard",the patients were divided into benign group and malignant group.The clinical data of the two groups were compared.The influencing factors of benign and malignant thyroid nodules were analyzed by multivariate Logistic regression.Receiver operating characteristic(ROC)curve was plotted,and the area under the curve(AUC)was used to evaluate the diagnostic efficiency of ultrasound features in benign and malignant thyroid nodules.Results:Among 126 patients,there were 55 cases in benign group(43.65%)and 71 cases in malignant group(56.35%).There were statistically significant differences in echo,aspect ratio,boundary,calcification,elasticity score,pattern of contrast enhancement and strain ratio of elasticity imaging between the two groups(P<0.001).Logistic regression analysis showed that aspect ratio,elasticity score and strain ratio of elasticity imaging were independent risk factors for benign and malignant thyroid nodules(P<0.05).ROC curve analysis showed that the AUC of aspect ratio,elasticity score and strain ratio of elasticity imaging in differential diagnosis of benign and malignant thyroid nodules were 0.802,0.757 and 0.741,respectively.The AUC of combined diagnosis was 0.891,and the sensitivity was higher than that of single index(P<0.05).Conclusion:Aspect ratio,elasticity score and strain ratio of elasticity imaging are risk factors for benign and malignant thyroid nodules of C-TIRADS Grades 4A and 4B.Combined detection of the three indexes can improve the sensitivity of differential diagnosis of benign and malignant thyroid nodules and have good diagnostic efficiency.
