摘要
先兆流产是妊娠早期常见并发症,临床以阴道出血、腰酸腹痛为主要表现,严重威胁母婴健康。何成瑶教授(以下简称何教授)基于“养精育胞”理论,提出本病的核心病机在于先后天之精亏虚、胞宫失养,以致冲任失固、阴阳失济、胎动不安。何教授指出“养精不唯补先天之精,更需温运三焦以助后天化源”,通过补脾益肾、温阳化气,使三焦枢机得畅,水谷精微得以输布胞宫;而“育胞”则通过调理冲任、固摄胞脉,使胞宫具备藏泻有序的生理功能。本文系统总结何成瑶教授基于养精育胞理论论治本病的学术思想,旨在为优化先兆流产的中医诊疗方案提供理论依据。
Threatened abortion is a common complication in early pregnancy,primarily characterized by vaginal bleeding,lumbago,and abdominal pain in clinical settings,posing a severe threat to the health of both the mother and the fetus.Based on the theory of"Nourishing Essence and Nurturing the Uterus",Professor He Chengyao proposed that the core pathogenesis of this disease lies in the deficiency of congenital and acquired essence,and the undernourishment of the uterus,leading to instability of the thoroughfare and conception vessels,imbalance of Yin and Yang,and fetal restlessness.She emphasized that"nourishing essence involves not only replenishing congenital essence but also warming and regulating the Triple Energizer to support the generation of acquired essence."By invigorating the spleen and tonifying the kidneys,as well as warming yang and transforming qi,the pivotal function of the triple energizer can be restored,enabling the transportation of nutrients to the uterus.Additionally,"nurturing the uterus"aims to regulate the thoroughfare and conception vessels and stabilize the uterine vessels,ensuring the uterus functions properly in terms of retention and discharge.This article systematically summarizes Professor He Chengyao's academic thoughts on the treatment of this disease based on the theory of nourishing essence and nurturing the uterus,aiming to provide a theoretical basis for optimizing Traditional Chinese Medicine(TCM)diagnosis and treatment protocols for threatened abortion.
