摘要
中医药治疗阳痿具有丰富的临床经验积累。通过系统梳理阳痿中医病名沿革,从先秦时期“不起”到《黄帝内经》“阴痿”,再到宋代“阳事不举”及明代“阳痿”确立,体现了对其认识的深化过程。病因可分为内因、外因与不内外因三大类,主要由外感六淫、内伤情志、房事过度、外伤、脏腑病变及年老体衰等因素所致。病机认识从《内经》的肾虚为主,发展到明清时期的肝郁、血瘀、湿热,再到现代的痰瘀互结、五脏失调等,日益全面。辨证施治包括脏腑论治、六经论治。中医治疗涵盖内治法、外治法及其他疗法。内治法遵循辨证施治原则,诸医家结合临床实践创立治法,如补肾滋阴、疏肝解郁、通补肝肾、活血化瘀、通络起痿等;外治法有针灸、推拿、药浴等;其他疗法包括情志疗法、导引、药膳等。中医药治疗阳痿具有显著的临床价值,但目前对病因病机的认识与分型施治标准尚未统一,且缺乏高质量的循证医学证据。未来研究应聚焦这些问题,推动中医药在阳痿治疗领域的创新发展,为临床治疗提供更坚实理论依据和实践参考,提升其临床疗效和应用价值。
Traditional Chinese medicine has accumulated rich clinical experience in the treatment of impotence.By systemati cally combing the evolution of impotence TCM disease names,from the pre-Qin"Buqi"to"Huangdi Neijing"and"Yinwei",to the Song Dynasty"Yangshi Buju"and the establishment of"impotence"in the Ming Dynasty,it reflects the continuous deep ening of its understanding.The causes are divided into three categories:internal causes,external causes and non-internal and external causes,which are caused by exogenous six evils,internal injuries,excessive sexual intercourse,trauma,zang-fu vis cera lesions and senile body failure.The pathogenesis is increasingly comprehensive,from kidney deficiency in"Neijing"to liver depression,blood stasis,damp-heat in Ming and Qing Dynasties,to modern phlegm and blood stasis,five internal organs dis orders and so on.Treatment based on syndrome differentiation includes treatment based on zang-fu organs and treatment based on six meridians.Traditional Chinese medicine treatment covers internal treatment,external treatment and other therapies.The internal treatment is based on the syndrome,and the doctors combine the practice to create a method,or tonifying kidney yin,or soothing liver depression,or tonifying liver and kidney,or promoting blood circulation and removing blood stasis,or dredging collaterals and flaccidity;external treatment includes acupuncture,massage,medicated bath,etc.;other therapies include e motional therapy,guidance,medicated diet,etc.The treatment of impotence with traditional Chinese medicine has shown signifi cant clinical value,but the understanding of etiology and pathogenesis and the standard of classification and treatment have not been unified,and there is a lack of high-quality evidence-based medical evidence.Future research should focus on these is sues,promote the innovative development of traditional Chinese medicine in the field of impotence treatment,provide a more sol id theoretical basis and practical reference for clinical treatment,and enhance its clinical efficacy and application value.
