摘要
目的:探讨尿布性皮炎风险评估量表联合标准结构化管理方案对新生儿的应用效果。方法:本研究采用系统文献检索与专家小组会议相结合的方式,构建了一套新生儿尿布性皮炎风险评估量表结合标准化的结构化管理方案。该方案涵盖高危因素风险评估、预防措施及临床管理三个核心模块。在研究方法上,我们采用非同期对照研究设计,对干预效果进行科学评价。病例选取2024年12月—2025年5月在江西省儿童医院新生儿重症监护室(NICU)的新生儿为研究对象,按照入院时间分组,2024年12月—2025年2月的50例患儿为对照组,给予常规护理;2025年3月—2025年5月的50例患儿为观察组,采用新生儿尿布性皮炎风险评估量表联合标准结构化管理方案护理。比较两组婴幼儿尿布性皮炎的发生情况、严重程度分布、症状缓解周期、住院天数及疼痛评估结果差异。结果:观察组患儿尿布性皮炎的发生风险低于对照组(P<0.05),且皮炎愈合更快(P<0.05)、住院时间更短(P<0.05)、疼痛程度更轻(P<0.05);然而,中重度皮炎的比例在两组间未见显著差异(P>0.05)。结论:尿布性皮炎风险评估量表联合标准结构化管理方案能有效改善新生儿尿布性皮炎的护理效果,规范护理人员的行为,提高护理质量。
Objective:To explore the application of diaper dermatitis risk assessment scale combined with standardized structured management in neonates.Methods:A standardized management protocol for neonatal diaper dermatitis,grounded in a risk assessment scale,was developed through a systematic literature review and expert panel discussions.The protocol encompasses three core components:risk factor assessment,preventive measures,and clinical management.A non-contemporaneous controlled study design was employed to evaluate the effectiveness of the intervention.The cases selected were newborns in the Neonatal Intensive Care Unit(NICU)of Jiangxi Provincial Children’s Hospital from December 2024 to May 2025.The cases selected were newborns in the Neonatal Intensive Care Unit(NICU)of Jiangxi Provincial Children’s Hospital from December 2024 to May 2025.They were grouped according to the admission time:50 children from December 2024 to February 2025 served as the control group and received routine care,while 50 children from March 2025 to May 2025 were assigned to the observation group.Neonatal Diaper Dermatitis Risk Assessment Scale combined with a standard structured management protocol were used for nursing care.The incidence of diaper dermatitis,severity distribution,symptom relief duration,length of hospital stay,and pain assessment results were compared between the two groups.Results:The observation group showed a significantly lower incidence of diaper dermatitis(P<0.05),a shorter healing time(P<0.05),reduced hospitalization duration(P<0.05),and lower pain scores(P<0.05)compared to the control group.However,the proportion of moderate-to-severe cases did not differ significantly between the groups(P>0.05).Conclusions:The integration of a diaper dermatitis risk assessment scale with a standardized structured management protocol significantly improves nursing outcomes for neonatal diaper dermatitis,standardizes nursing practices,and improves overall care quality.
