摘要
目的:检索、整理并汇总肝癌围术期下肢深静脉血栓管理的相关证据,为肝癌围术期下肢深静脉血栓管理的临床实践提供参考。方法:基于“6S”循证金字塔模型全面检索BMJ Best Practice、UpToDate、Cochrane Library、PubMed和中国生物医学文献数据库等。检索时限为2019年1月—2024年12月。检索内容包括指南、系统评价、证据总结和临床实践。由3名经过培训的研究人员独立使用标准化工具进行质量评估,然后按照既定的共识提取和综合证据。结果:最终纳入13篇文献,包括8篇指南、2篇系统评价、2篇证据总结和1篇临床实践。最终的证据涵盖了风险因素的控制、评估、基础预防、机械预防、药物预防、健康教育、团队培训7个维度共21条最佳证据。结论:肝癌围术期下肢深静脉血栓管理需要综合考虑手术类型、个体风险以及术后恢复情况,通过多层次的预防和治疗措施,优化围术期深静脉血栓的管理策略,有效降低围术期下肢深静脉血栓的发生率,改善病人预后。
Objective:To retrieve,organize,and synthesize evidence on the management of perioperative deep vein thrombosis(DVT)of the lower extremities in patients with liver cancer,providing a reference for clinical practice in this area.Methods:A comprehensive search was conducted using the"6S"evidence‐based pyramid model across BMJ Best Practice,UpToDate,Cochrane Library,PubMed,CBM and so on.The search period was from January 2019 to December 2024.Search content included guidelines,systematic reviews,evidence summaries,and clinical practice.A total of 3 trained researchers independently assessed quality using standardized tools,then extracted and synthesized evidence according to established consensus.Results:A total of 13 articles were ultimately included,comprising 8 guidelines,2 systematic reviews,2 evidence summaries,and 1 clinical practice.The final evidence encompassed 21 pieces of best evidence across 7 dimensions,including risk factor control,assessment,primary prevention,mechanical prevention,pharmacological prevention,health education,and team training.Conclusion:Management of perioperative DVT of lower extremities in patients with liver cancer requires comprehensive consideration of surgical type,individual risk,and postoperative recovery.Implementing multi‐tiered prevention and treatment measures optimizes perioperative DVT management strategies,effectively reducing perioperative incidence of DVT of lower extremities and improving patient outcomes.
