摘要
老年友善型医院医养结合创新发展是积极应对人口老龄化和满足老年人健康服务需求的重大举措。其必要性在于人口老龄化加剧与失能高龄化的现实倒逼、老年健康需求复杂化与服务体系碎片化的矛盾凸显、资源配置低效与老龄化成本攀升的双重压力、政策导向与积极老龄化战略的实施要求、社会价值与民生需求的深层呼应。老年友善型医院医养结合创新发展在实践中面临诸多困境,突出表现在医疗和养老服务资源整合存在明显不足、基础设施与技术应用滞后于发展需求、老年群体需求认知与支付能力矛盾突出、社会力量参与医养结合的壁垒较高、文化观念和社会支持体系缺失等多个方面。基于此,提出强化医疗与养老资源深度整合、加速基础设施适老化改造与智慧化升级、破解需求认知与支付能力矛盾、降低社会力量参与医养结合的壁垒、重塑文化观念与构建多元社会支持网络等破解路径。
Innovative development of integrated medical and elderly care in age-friendly hospitals is a major initiative to actively respond to population aging and meet the health service needs of older adults.Its necessity lies in multiple factors,including the intensified aging of the population and the growing prevalence of disability among the oldest-old,the increasing complexity of older adults’health needs alongside fragmentation of the service system,the dual pressures of inefficient resource allocation and rising aging-related costs,the requirements of policy orientation and the implementation of the active aging strategy,and the deep resonance between social values and people’s livelihood needs.In practice,innovative development of integrated medical and elderly care in age-friendly hospitals faces numerous dilemmas,most notably insufficient integration of medical and elderly care resources,infrastructure and technological applications lagging behind development needs,prominent contradictions between older adults’demand awareness and payment capacity,high barriers for social forces to participate in integrated care,and deficiencies in cultural perceptions and social support systems.Accordingly,this study proposes pathways to address these challenges,including strengthening deep integration of medical and elderly care resources,accelerating age-friendly renovation and intelligent upgrading of infrastructure,resolving the mismatch between demand awareness and payment capacity,lowering barriers for social forces to participate in integrated care,reshaping cultural perceptions,and building diversified social support networks.
