摘要
目的对比传统冷牙胶侧方加压法(TCLC)与改良冷牙胶侧方加压法(MCLC)在慢性根尖周炎治疗中的临床疗效及复发率差异。方法回顾性纳入2023年1月-2024年1月百色市妇幼保健院收治的慢性根尖周炎患者60例,按随机数表法分为TCLC组(传统冷牙胶侧方加压法进行根管充填)与MCLC组(改良冷牙胶侧方加压法进行根管充填),每组30例。对比两组患者治疗后12个月的影像学愈合状况、根管填充质量及复发率。结果治疗后12个月,MCLC组影像学愈合率高于TCLC组(P<0.05);MCLC组根管填充质量更优,密度缺陷更少,长度达标率高于TCLC组(P<0.05);MCLC组复发率明显低于TCLC组(P<0.05)。结论改良冷牙胶侧方加压法在慢性根尖周炎治疗中可获得更优的短中期疗效,降低复发风险,临床可优先选择改良冷牙胶侧方加压法,以提升治疗成功率与长期稳定性。
Objective To compare the clinical efficacy and recurrence rates of traditional lateral condensation of cold gutta-percha(TCLC)versus modified lateral condensation of cold gutta-percha(MCLC)in treating chronic apical periodontitis.Methods A retrospective study enrolled 60 patients with chronic apical periodontitis treated at Guangxi Baise Maternal and Child Health Care Hospital from January 2023 to January 2024.Patients were randomly assigned using a random number table to either the TCLC group(root canal filling with traditional lateral condensation of cold gutta-percha)or the MCLC group(root canal filling with modified lateral condensation of cold guttapercha),with 30 patients in each group.The groups were compared regarding radiographic healing status,root canal filling quality,and recurrence rate at 12 months post-treatment.Results At 12 months post-treatment,the MCLC group demonstrated a significantly higher radiographic healing rate than the TCLC group(P<0.05).The MCLC group exhibited superior root canal filling quality with fewer density defects and a higher rate of achieving adequate length compared to the TCLC group(P<0.05).The recurrence rate in the MCLC group was significantly lower than that in the TCLC group(P<0.05).Conclusion The modified cold lateral condensation technique yields superior short-and medium-term outcomes in treating chronic apical periodontitis,reduces recurrence risk,and should be prioritized clinically to enhance treatment success and long-term stability.
