摘要
目的探讨“互联网+”家庭肠内营养管理方案在脑卒中患者中的应用,并验证其对患者营养状况、并发症发生率及神经功能恢复的影响。方法采用类实验研究设计,将2024年1—6月张家港市第一人民医院神经外科收治的52例行肠内营养支持的脑卒中患者设为对照组,2024年7—12月张家港市第一人民医院神经外科收治的55例行肠内营养支持的脑卒中患者为观察组,对照组实施常规性护理干预,观察组在对照组基础上实施为期3个月的“互联网+”家庭肠内营养管理方案。干预前后对两组患者前白蛋白(PA)、转铁蛋白(TRF)、血红蛋白(Hb)、改良Rankin量表(mRS)评分、美国国立卫生研究院卒中量表(NIHSS)评分、日常生活能力(Barthel指数)评分及肠内营养并发症发生率进行评价。结果观察组患者实施“互联网+”家庭肠内营养管理方案后营养指标(PA、TRF、Hb)较对照组有所改善(P<0.05)。观察组患者实施“互联网+”家庭肠内营养管理方案后Barthel指数评分较对照组高,mRS评分、NIHSS评分较对照组低,经比较差异有统计学意义(P<0.05)。观察组患者实施“互联网+”家庭肠内营养管理方案后肠内营养相关并发症发生率低于对照组,经比较差异有统计学意义(P<0.05)。结论“互联网+”家庭肠内营养管理方案能有效提升脑卒中患者的家庭营养管理效果,促进患者康复,其远程视频监督可为居家营养管理提供有效解决方案。
Objective To explore the application of"Internet+"home enteral nutrition management program for stroke patients and verify its effects on patients'nutritional status,complication rates,and neurological function recovery.Methods Using a quasi-experimental research design,52 stroke patients receiving enteral nutrition support admitted to the Department of Neurosurgery at Zhangjiagang First People's Hospital from January to June 2024 were assigned to the control group,while 55 stroke patients receiving enteral nutrition support admitted from July to December 2024 were assigned to the observation group.The control group received conventional nursing interventions,while the observation group received a 3-month"Internet+"home enteral nutrition management program in addition to the conventional interventions.Prealbumin(PA),transferrin(TRF),hemoglobin(Hb),Modified Rankin Scale(mRS)score,National Institutes of Health Stroke Scale(NIHSS)score,activity of daily living(Barthel index)score,and the incidence of enteral nutrition complications were evaluated in both groups of patients before and after the intervention.Results After implementing the"Internet+"home enteral nutrition management program,patients in the observation group showed improved nutritional indicators(PA,TRF and Hb)compared to the control group(P<0.05).After implementing the"Internet+"home enteral nutrition management program,the Barthel index score in the observation group was higher than that in the control group,the scores of mRS and NIHSS in the observation group were lower than those in the control group,the differences were statistically significant after comparison(P<0.05).After implementing the"Internet+"home enteral nutrition management program,the incidence of enteral nutrition-related complications in the observation group was lower than that in the control group,the difference was statistically significant after comparison(P<0.05).Conclusion The"Internet+"home enteral nutrition management program can effectively improve the effectiveness of home nutrition management for stroke patients and promote their recovery.Its remote video supervision provides an effective solution for home-based nutrition management.
