摘要
目的探讨清单式集束化护理策略在胃肠外科术后下肢深静脉血栓(DVT)预防中的应用效果。方法选取2022年1月至2023年1月南昌大学附属康复医院80例接受胃肠外科手术治疗的患者作为研究对象,按照随机数字表法分为对照组(40例)和干预组(40例)。对照组实施常规护理,干预组实施清单式集束化护理策略干预,两组均干预至出院。比较两组下肢DVT发生率、下肢周径、下肢深静脉血流速度、住院时间、生活质量、并发症发生率及护理满意度。结果干预组下肢DVT发生率以及并发症总发生率均低于对照组,差异有统计学意义(P<0.05)。干预组下肢周径和住院时间均短于对照组,下肢深静脉血流速度快于对照组,差异有统计学意义(P<0.05)。干预组慢性静脉功能不全生活质量量表(CIVIQ)评分均高于对照组,差异有统计学意义(P<0.05)。干预组护理满意度各项评分均高于对照组,差异有统计学意义(P<0.05)。结论清单式集束化护理策略对胃肠外科术后下肢DVT的预防效果确切,不仅能够缩短患者下肢周径,减轻下肢肿胀,延长下肢深静脉血流速度,对降低下肢DVT发生率有积极作用,还能够减少术后肺部感染、切口感染等发生概率,提高患者的生活质量,从而获得更高的护理满意度,值得在胃肠外科手术中推广应用。
Objective To explore the application effect of checklist-based bundle nursing strategy in the prevention of deep vein thrombosis(DVT)in the lower limbs after gastrointestinal surgery.Methods A total of 80 patients who underwent gastrointestinal surgery in Rehabilitation Hospital Affiliated to Nanchang University from January 2022 to January 2023 were selected as the study subjects,and they divided into the control group(40 cases)and the intervention group(40 cases)by random number table method.The control group received routine nursing care,while the intervention group was managed with a checklist-based bundled nursing strategy.Both groups received their respective interventions until discharge.The incidence of lower limb DVT,lower extremity circumference,blood flow velocity of lower limb deep veins,length of hospital stay,quality of life,incidence of complications,and nursing satisfaction were compared between the two groups.Results The incidence of lower limb DVT and the total complication rate in the intervention group were lower than those of the control group,with statistically significant differences(P<0.05).The lower limb circumference and hospital stay in the intervention group were shorter than those of the control group,and the deep venous blood flow velocity was faster than that of the control group,and the differences were statistically significant(P<0.05).The chronic venous insufficiency questionnaire(CIVIQ)scores in the intervention group were higher than those of the control group,with statistically significant differences(P<0.05).All nursing satisfaction scores in the intervention group were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion The checklist-based bundled nursing strategy demonstrates a definitive preventive effect on lower limb DVT in gastrointestinal surgery patients.It not only helps reduce lower limb circumference,alleviate lower limb swelling,and enhance deep venous blood flow velocity,thereby actively contributing to the reduction of DVT incidence,but also decreases the probability of postoperative complications such as pulmonary infection and surgical site infection.Furthermore,it improves patients'quality of life,leading to higher nursing satisfaction.This approach is worthy of broader promotion and application in gastrointestinal surgical practice.
