摘要
目的:引进爱丁堡产后抑郁量表(EPDS)并进行本土化,在产妇群体中验证其效度和信度。方法:对EPDS英文版进行翻译、回译、专家委员会审议、预调查,形成EPDS中文版。选取湖南省两所妇幼保健院产后病房的产妇300人,随机选取100例进行探索性因子分析,其余200例进行验证性因子分析。使用EPDS中文版对总样本进行筛查,随后由实施者以DSM-IV诊断标准为参照进行诊断性访谈。结果:探索性因子分析共提取2个公因子,因子负荷分别为0.48~0.79和0.47~0.84,累积贡献率为55.33%;验证性因子分析结果显示模型拟合较好(χ^(2)/df=2.09,RMR=0.02,RMSEA=0.07,GFI=0.94,NFI=0.88,CFI=0.93,IFI=0.94,TLI=0.91)。EPDS中文版的Cronbachα系数为0.83,间隔24h的重测信度为0.93。根据诊断性访谈结果,确定使用EPDS中文版筛查产后抑郁症的最佳临界值为14/15分,此时灵敏度为85.7%、特异度为94.5%、阳性预测值为27.1%、阴性预测值为99.6%;AUC为0.94(95%CI:0.87~1.00)。结论:爱丁堡产后抑郁量表中文版具有良好的效度和信度,可作为筛查产后抑郁症的有效工具,但需关注假阳性较高的问题。
Objective:To adapt and validate the Edinburgh Postnatal Depression Scale(EPDS)for use among postpartum women,in China.Methods:The English version of the EPDS was translated,back-translated,reviewed by an expert panel,and pretested to develop the Chinese version.A total of 300 postpartum women were recruited from two maternal and child health hospitals in Hunan Province.Exploratory factor analysis(EFA)was conducted in a randomly selected 100 participants,while confirmatory factor analysis(CFA)was performed on the remaining 200.The participants completed the Chinese version of the EPDS,followed by diagnostic interviews based on DSM-IV diagnostic criteria.Results:EFA extracted two common factors,with factor loadings ranging from 0.48 to 0.79 and 0.47 to 0.84,respectively,and a cumulative variance contribution rate of 55.33%.CFA results indicated a good model fit(χ^(2)/df=2.09,RMR=0.02,RMSEA=0.07,GFI=0.94,NFI=0.88,CFI=0.93,IFI=0.94,TLI=0.91).The Cronbach′sαof the Chinese version of the EPDS was 0.83,the test-retest reliability was 0.93.Based on the results of diagnostic interviews,the optimal cutoff score for screening postpartum depression using the Chinese version of the EPDS was identified as 14/15,yielding a sensitivity of 85.7%,specificity of 94.5%,positive predictive value of 27.1%,and negative predictive value of 99.6%.The area under the curve(AUC)was 0.94(95%CI:0.87-1.00).Conclusion:The Chinese version of the Edinburgh Postnatal Depression Scale(EPDS)demonstrates good validity and reliability and could be used as an effective tool for screening postpartum depression.However,attention should be paid to the issue of high false-positive rates.
