摘要
目的:调查精神分裂症患者病耻感情况,分析相关因素,为探讨针对性病耻感消除策略提供参考。方法:采用方便抽样法,选取18~60岁符合ICD-10精神分裂症诊断标准且临床稳定3个月以上的患者195例。使用一般人口学调查表、精神疾病内在病耻感量表(ISMI)和社会支持评定量表(SSRS)以收集患者信息并评估其病耻感严重程度及社会支持水平。用有序logistic回归分析精神分裂症患者病耻感的相关因素。结果:76.4%(149/195)的精神分裂症患者存在病耻感,其中中重度以上的占26.6%。社会支持良好(OR=0.92,P<0.01)、男性(OR=0.49,P <0.05)、独居(OR=0.20,P<0.05)、病前有工作(OR=0.41,P<0.05)是患者病耻感的独立保护因素;年长(OR=1.04,P<0.05)、无配偶(OR=2.31,P<0.05)、有子女(OR=2.58,P<0.05)是患者病耻感的独立危险因素。结论:精神分裂症患者病耻感与社会支持水平、年龄、性别、独居情况、婚姻、子女情况及病前工作情况等因素相关。
Objective:To investigate the level of internalized stigma among patients with schizophrenia in the stable phase and to identify its associated factors,providing evidence for targeted stigma reduction strategies.Met-hods:A total of 195 patients aged 18 to 60 years old who met the ICD-10 diagnostic criteria for schizophrenia and had been clinically stable for more than 3 months were recruited.A general demographic questionnaire,the Internalized Stigma of Mental Illness(ISMI)Scale,and the Social Support Rating Scale(SSRS)were used to assess the severity of internalized stigma and the level of social support of the patients with schizophrenia.Ordinal logistic regression was used to identify the factors associated with internalized stigma among the patients with schizophrenia.Results:Internalized stigma was reported by 76.4%(149/195)of patients,with 26.6%experiencingmoderate to severe levels.Strong social support(OR=0.92,P<0.01),malegender(OR=0.49,P<0.05),living alone(OR=0.20,P<0.05)and having been employed prior toillness(OR=0.41,P<0.05)was identified as independent protective factors.Older age(OR=1.04,P<0.05),being single(OR=2.31,P<0.05)and having children(OR=2.58,P<0.05)were identified as independent risk factors.Conclusion:Internalized stigma is highly prevalent among stablephase patients with schizophrenia and is influenced by multiplefactors such as the level of social support,age,gender,living alone,marital status,family circumstances and pre-illnes employment.
