摘要
女性癌症负担持续加重,已成为全球公共卫生与性别公平的重大挑战。亚洲承载了全球近一半的癌症病例和过半死亡病例,中国作为该地区最大的发展中国家,呈现出高疾病负担与健康不平等并存的典型局面。在癌症防治体系中,女性不仅是患者,还承担着照护者和卫生工作者的角色,却长期受到医疗服务可及性不足、无偿照护责任沉重及决策层代表性缺失等系统性障碍的制约,加剧了社会结构性不平等。在此背景下,《女性、权力与癌症:柳叶刀重大报告》作为里程碑式的系统性成果,从交叉女权主义视角揭示女性在癌症预防、诊疗与照护中的多重障碍,并提出涵盖数据收集、立法政策、科研创新、健康系统改革、经济评价与照护劳动价值等十项优先行动建议。本文综述该《报告》的核心发现与优先行动建议,结合《健康中国行动(2019—2030年)》,分析其与我国健康政策的契合性及启示,并立足我国文化背景提出癌症护理领域的可操作性建议,以推动我国高质量、性别公平的肿瘤护理实践,为亚洲及全球女性癌症防治提供可借鉴的路径。
The escalating burden of female cancers has become a critical challenge for both global public health and gender equity.Asia accounts for nearly half of the world′s cancer cases and more than half of cancer-related deaths.Within this region,China,as the largest developing country,exemplifies the coexistence of a high disease burden and entrenched health inequities.Within cancer control systems,women serve not only as patients but also as caregivers and health professionals.However,persistent systemic barriers such as inadequate access to care,heavy unpaid caregiving burdens,and limited representation in leadership-have compounded social structural inequalities.Against this backdrop,[WTBX]Women,Power,and Cancer:A Lancet Commission[WTBZ]represents a landmark and systematic achievement that,from an intersectional feminist perspective,exposes the multifaceted obstacles women face across cancer prevention,treatment,and care.The Commission also outlines ten priority actions encompassing data collection,legislation and policy,scientific research and innovation,health system reform,economic evaluation,and the recognition of the value of care labor.This article synthesizes the Commission′s key findings and priority recommendations,analyzes their synergies and implications for China′s health policy in conjunction with the"Healthy China Initiative(2019-2030)",and proposes actionable strategies for cancer are grounded in China′s sociocultural context.These efforts aim to advance high-quality,gender-equitable oncology nursing practice in China,thereby contributing actionable pathways for cancer prevention and control for women in Asia and globally.
