摘要
目的 检索、评价并汇总患儿经外周静脉输注非抗肿瘤药物外渗预防和处理的最佳证据,为临床实践提供参考。方法 系统检索计算机决策支持系统、指南网站、专业协会网站及数据库中关于患儿经外周静脉输注非抗肿瘤药物外渗预防和处理的相关文献,检索时限为建库至2025年3月,纳入临床决策、指南、系统评价、专家共识、证据总结。由2名研究人员独立筛选文献,评价纳入文献的质量,并结合专业知识进行证据的提取、翻译与整合。结果共纳入9篇文献,包括3篇指南、3篇系统评价、2篇专家共识及1篇证据总结,从外渗的预防、评估及处理3个方面总结了30条证据。结论 该研究总结的患儿经外周静脉输注非抗肿瘤药物外渗预防、评估与处理的最佳证据可为临床实践提供实质性参考,建议医护人员根据药物性质及风险类别、外渗损伤程度等具体情况选择性应用最佳证据,以保障外周静脉输液安全。
Objective To retrieve,evaluate,and synthesize the best available evidence on the prevention and management of non-anticancer drugs'extravasation in pediatric peripheral intravenous therapy,and to provide an evidence-based basis for clinical practice.Methods A systematic search was conducted across computerized decision support systems,guideline websites,professional association websites,and databases for evidence related to the prevention and management of non-anticancer drugs'extravasation in pediatric peripheral intravenous therapy.The search encompassed the literature from database inception to March 2025.This study included clinical decision-making,guidelines,systematic reviews,expert consensuses,and evidence summaries.Totally 2 researchers independently screened the literature,assessed the quality,extracted,translated,and synthesized evidence through professional judgment.Results A total of 9 articles were included,consisting of 3 guidelines,3 systematic reviews,2 expert consensuses,and 1 evidence summary.The study synthesized 30 best practices across 3 aspects,namely prevention,assessment,and management of extravasation.Conclusion This study systematically retrieved and summarized the best evidence of prevention,assessment,and management of extravasation of non-anticancer drugs in pediatric peripheral intravenous therapy to provide substantive references for clinical practice.Healthcare professionals are advised to selectively apply these evidence-based recommendations,considering factors such as the drug's properties,risk category,and the severity of extravasation injury,to ensure the safety of pediatric peripheral intravenous infusion.
