摘要
目的 通过追踪方法学对PICC置管质量进行追踪检查,建立青海高原地区PICC质量管理方法 ,并检验其效果。方法 2025年3—6月,西宁市某三级甲等医院根据追踪方法学的理论基础,确定追踪路线及考核内容,应用根因分析法分析PICC置管问题及原因,制订具体的改进措施。比较改进前后护士置管操作时间、一次性置管成功率、PICC并发症(自PICC置管后3个月)及医生、患者的满意度。结果 基于追踪方法学的PICC质量管理实施后,与改进前相比,置管操作时间明显缩短,医生(医护配合、专业能力)、患者(专业知识、操作技术、交流沟通、舒适体验)的满意度得到改善,PICC相关并发症发生率下降至6.7%,差异均具有统计学意义(P<0.05)。结论 通过追踪方法学开展青海高原地区医院PICC质量管理具备可行性和高效性,针对核心问题的精准整改有助于提升护理质量,保障患者安全。
Objective The quality of PICC catheterization was tracked and checked by the tracking methodology,and the PICC catheterization quality management method was established and its effect was tested.Methods From March to June 2025,a tertiary grade-A hospital in Xining City determined the tracking route and assessment content based on the theoretical framework of tracer methodology.The root cause analysis method was used to analyze the causes;specific improvement measures were formulated;the nurse catheter placement operation time,one-time catheter placement success rate,PICC complications(3 months after PICC insertion),and the satisfaction of doctors and patients were compared before and after the study.Results After implementation,the time of catheterization was significantly shortened,the satisfaction of doctors(medical and nursing cooperation,professional ability)and patients(professional knowledge,operation skills,communication and comfortable experience)were significantly improved,and the incidence of complications was significantly reduced to 6.7%,and the difference was statistically significant compared with those before implementation(P<0.05).Conclusion The tracking methodology is feasible and efficient to the quality management of PICC catheterization in the Qinghai Plateau.Precise rectification targeting core problems helps to improve nursing quality and ensure patient safety.
